Ray Breslin, Sylvester Stallone, runs his security company with a mix of steady success and growing pressure. His senior team includes Hush [50 Cent] and Abigail Ross [Jaime King], along with newcomers Shu Ren [Huang Xiaoming], Jasper Kimbral [Wes Chatham], and Luke Walken [Jesse Metcalfe], who operate in the field to handle dangerous tasks. During a hostage rescue mission in Chechnya, Kimbral’s reliance on his computer algorithms drives him to deviate from the plan, and a hostage is shot and later dies from her wounds; Breslin fires him, reshaping the team’s dynamics and trust.

A year after that setback, Shu Ren [Huang Xiaoming] is recruited to protect his cousin Yusheng Ma [Chen Tang], who is about to unveil groundbreaking satellite communications technology from his telecommunications company. Ruscho, a rival corporation, targets them, and after a party in Bangkok, Shu defends Yusheng from masked assailants, but both are stunned with tasers and kidnapped. Shu wakes up in an unknown arena where inmates are forced to fight; he defeats another prisoner and then encounters Kimbral, who explains that the prison is called HADES, where he has been held for months. Breslin, Hush, Abigail, and Luke race to locate Shu after his disappearance alongside Yusheng.

Drawing on Breslin’s training, Shu meets the warden Gregor Faust, Titus Welliver who calls himself the “Zookeeper,” and learns that Faust wants Yusheng’s patents in exchange for their release. Yusheng reveals that he buried the technology, fearing it could be hijacked to launch nuclear weapons, and Shu and Yusheng vow to escape. Breslin infers that HADES is funded by the same mysterious organization behind the Tomb and seeks help from an old contact, Trent DeRosa [Dave Bautista]. While chasing a lead on Shu’s whereabouts, Luke is captured and transferred to HADES.

Shu befriends the prison cook and learns the layout shifts each night, marked by a pencil on the floor, and Kimbral hints at a hacker known as Count Zero who leads a group called Legion. Shu earns Count Zero’s trust, gaining critical information, but Count Zero’s identity is revealed in the process. The next day, Shu, Luke, and Kimbral are brought in for questioning and forced to watch Count Zero’s apparent execution. Kimbral then reveals that he runs HADES; since leaving Breslin’s team, he built HADES to prove his algorithms work and to imprison people with sensitive intel to squeeze information from them, selling it to the highest bidder. He vows to make Shu and Yusheng’s lives miserable unless the patent is handed over, though Faust cautions him to focus on the prize.

Breslin and DeRosa uncover clues linking Kimbral to HADES. Knowing Kimbral’s vendetta, Breslin allows himself to be captured and sent inside HADES. With a hidden comm device in his tooth, Breslin stays in contact with Hush while working to breach the prison from within. Shu, Luke, and Yusheng rally a plan with help from the inmates and DeRosa on the outside, aiming to disable the prison’s defenses. They manage to shut down Galileo, the automated defense system, and disable security cameras, then push toward the control room. The Zookeeper orders an armed response, and chaos erupts as several inmates are killed and the group is scattered. DeRosa tracks the prison from the outside and closes in.

Yusheng overrides the power system, leading DeRosa to the prison’s entrance and allowing a bold breach. Shu and Luke converge on the control room as the exit becomes reachable. In a knife fight, Shu kills the Zookeeper, and Breslin squares off with Kimbral in a brutal confrontation. DeRosa arrives to rescue Luke from the guards, and together Breslin and DeRosa disable Galileo for good. Shu and Yusheng slip through the exit, ultimately being evacuated by Abigail and Hush. In the aftermath, Breslin is contacted by the shadowy group behind HADES and pledges to pursue them and expose their operation to the world.
