Pittsburgh, 1999 – Gemma, [Amelia Eve], a young girl, comes home from school to a mother Ingrid, [Laura Gordon], who is constantly tired and barely notices her. That night, Gemma overhears a disturbance downstairs and finds Ingrid in the kitchen wielding a knife. Ingrid urges Gemma to “Watch me” before she slams her own head against the wall, leaving Gemma trapped as blood seeps under the door.

Two decades pass. Gemma, [Amelia Eve], is now a dentist who shares a home with her daughter Maya, [Island Austin], and they’ve kept Ingrid’s tragedy largely to themselves. Gemma is dating Nick Mendoza, [Brandon Perea], a dedicated cop. On the way to school, they encounter three unsettling neighbors—Lester, [Joseph Lopez], Edna, [Robina Beard], and Patsy, [Susan Gray]—whose unnerving smiles and whispered greetings unsettle Gemma and Maya as they pace the sidewalk.

That night, Gemma’s world grows even darker when she becomes trapped in a nightmarish pillow fort in which the same three elderly figures loom in the shadows. The moment she dares to relax, Lester seizes Gemma’s hand and plants a pin in it. She finally claws her way out, but the scare lingers, and she immediately senses that something has followed her back from the darkness. At a clinic appointment, Gemma treats Lester and discovers something alarming: his mouth contains two sets of teeth, and the second set bites her fingers, triggering a fall that leaves her dazed and disoriented. When she awakens, she finds herself in The Further, a shadowy realm inhabited by demons and memories alike.

Saved from the demonic crowd by Elise Rainier, [Lin Shaye], Gemma learns the history of The Further and the famous Lambert family’s past struggles with this eerie dimension. Elise entrusts her with a book and points her toward Claire, [Maisie Richardson-Sellers], a tattoo parlor conduit who can facilitate communication with Elise’s spirit. Claire explains that Cyrus, [Sam Spruell], a new, menacing demon, has learned about Gemma’s ability to move things between The Further and the living world—a role Elise calls a “courier”—and intends to exploit it to reopen the living world to demons. The elderly trio—Lester, Edna, and Patsy—serve as acolytes guarding a newly opened door between The Further and the living world.

Gemma experiences a vision in which the dead rise and threaten her circle, including Nick. She wakes, distressed, and warns Nick that Cyrus’s plan is real. To protect Maya, Gemma keeps her daughter with a neighbor, Liz, [Zahra Newman], while she and Nick prepare for the coming confrontation. The plan to stop Cyrus hinges on Gemma stepping back into The Further with Elise’s help to reach the red door, but first they must lure the acolytes away from it.

In a tense sequence, Nick awakens the acolytes inside the Sullivan house in an effort to shield Gemma. Gemma and Elise confront Ingrid’s spirit, revealing Ingrid’s role as a courier who almost allowed Cyrus to escape into the living world—until she sacrificed herself to prevent it, closing the door for Gemma’s sake. Ingrid’s apparition reconciles with Gemma, giving her resolve. Yet Cyrus’s plan escalates when he finally pulls Maya from the world into his realm and uses her as a vessel to summon more demons, including the Bride in Black. Liz is slain as Cyrus rips open a path for his followers.

With the house overrun by demonic forces, Nick manages to evacuate Maya, while Elise temporarily possesses Lester to help the family escape. Cyrus murders the elderly acolytes and presses toward Gemma, who confronts him with a daring, desperate plan. Gemma channels Elise, returning to The Further to reach the red door, while Nick lures the demons away from the doorway. She manages to teleport the entire house filled with demons back into The Further, then traps them by forcing the scene to mislead them about their location.
