Tony Stark (Robert Downey Jr.) finds himself reminiscing about a New Year’s Eve party in 1999, where he encounters scientist Maya Hansen. Hansen, played by Rebecca Hall, is the pioneer behind Extremis, an innovative regenerative treatment designed to facilitate recovery from severe injuries. This gathering also marks the first meeting between Stark and Yinsen, portrayed by Shaun Toub.
At the party, a disabled scientist named Aldrich Killian (Guy Pearce) extends an invitation to join his company, Advanced Idea Mechanics (AIM), but Stark, in his typical brash manner, dismisses him arrogantly. After sending Killian off to the rooftop with a fleeting promise to return, Stark forgets about him, leaving Killian in the cold. This moment subsequently fuels Killian’s intense animosity towards Stark.
Over time, Hansen’s work suffers a setback as her treatment exhibits a dangerous flaw, leading to violent explosions in treated plants. Fast forward to the present, Stark grapples with panic attacks stemming from his traumatic experiences during the New York alien invasion. This turmoil results in him crafting multiple Iron Man suits, straining his relationship with girlfriend Pepper Potts (Gwyneth Paltrow). The latest models of his suits are controlled via micro-implants, allowing him to summon them remotely.
The tension escalates as a series of bombings attributed to a terrorist named the Mandarin leaves authorities baffled—despite nine attacks, no bomb fragments can be traced back to any crime scene. Meanwhile, Killian reaches out to Potts, demonstrating the revolutionary Extremis technology. Intriguingly, Potts has a prior connection with Killian and seems to harbor feelings for him. Killian claims that Extremis can reprogram human DNA, but Potts, wary of its potential dangers, declines his collaboration offer.
When Stark’s security chief, Happy Hogan, played by Jon Favreau, becomes seriously injured in a blast allegedly orchestrated by the Mandarin, the stakes are raised. Following Happy’s comatose state after the attack, Stark issues a public threat to the Mandarin, who retaliates by obliterating Stark’s residence.
In his pursuit of the truth, Stark, directed by his AI, JARVIS (Paul Bettany]), finds himself in rural Rose Hill, Tennessee, which has also experienced a similar explosion linked to the Mandarin. Unbeknownst to the world, Stark is presumed dead; he teams up with a clever ten-year-old named Harley (Ty Simpkins), determined to probe into the local explosion’s connection to the Mandarin’s operations.
Their investigation unveils the alarming truth: the bombings were caused by veterans afflicted with Extremis, leading to explosive reactions that cover the project’s failures under a shroud of terror. Stark finds himself under attack by Extremis agents, including Ellen Brandt (Stephanie Szostak) and Eric Savin (James Badge Dale), and learns first-hand about the flawed technology.
In a stunning twist, Stark infiltrates the Mandarin’s lair, only to unearth that this so-called terrorist is nothing more than Trevor Slattery, a clueless British actor manipulated by Killian, who now reveals himself as the true mastermind. Killian’s plan includes kidnapping Potts and using her as leverage to force Stark into aiding him with Extremis.
The final showdown unfolds when Killian launches an audacious attack on President Ellis aboard Air Force One. Stark, using newly recovered suits, manages to save Ellis and engages in a fierce battle against Killian, who intends to broadcast his murderous intent live on television. With the help of Potts, who survives the Extremis enhancements, they confront Killian together, leading to a climactic victory.
In the aftermath, Stark resolves to destroy his remaining armors as a gesture of commitment to Potts. With the immediate threats neutralized and a newfound focus on their relationship, Stark ponders his identity, tossing away the arc reactor that once defined him. In a whimsical twist, the story concludes where it began—Stark awakening Dr. Bruce Banner from a slumber, signaling the continuation of their adventures.
