Set in a private mental institution, Chestnut Lodge in Rockville, Maryland, the film follows Vincent Bruce, Warren Beatty, a troubled ex-soldier training to be an occupational therapist, who becomes fascinated by the seductive, artistic, schizophrenic patient Lilith Arthur, [Jean Seberg], whose mind is as alluring as it is dangerous. Vincent works to help Lilith emerge from her seclusion, and soon their world narrows to moments spent away from the institutional grounds, first for a day in the country and then on occasional excursions in which she is alone with him. The pull between them grows into something intimate, and Lilith begins to seduce him as he grapples with mounting feelings he knows are inappropriate for a patient and a trainee therapist.

During one carnival outing, Vincent witnesses Lilith behaving in a troubling, inappropriate way toward a young boy, an event that casts a shadow over their budding relationship. After that outing, the pair sleep together and begin meeting regularly, with Vincent becoming increasingly protective and possessive.
