Dr. Guy Luthan's ordinary night shifts as a New York emergency room physician took an unexpected turn when he encountered a mysterious patient: a homeless individual who wore a hospital wristband from a facility unknown to him, referenced a drug unfamiliar to the medical community, and displayed inexplicable symptoms, including a heart rate that defied the norm. As the man's condition deteriorated, Guy was left reeling by his sudden death, only to discover that all records of the patient had vanished along with his body. When he tried to investigate further, Guy was met with resistance from his superiors, who instructed him to abandon the case.

As Guy delved deeper into the mystery, his personal and professional life became increasingly turbulent. His home was breached, and evidence of cocaine was planted near his bedside, leading to a series of events that would forever alter the course of his existence. The authorities arrested and convicted him, stripping him of his medical license, his reputation, and his social connections. In desperation, Guy sought refuge among the homeless community, who revealed to him the existence of an underground network that had been ravaged by the same mysterious force.

This chance encounter led Guy to a network of makeshift homes, where he discovered that one of its residents was, in fact, the same patient whose death had sparked his investigation. The thread of fate then guided him towards an organization led by neurosurgeon Dr. Lawrence Myrick, who claimed to be on the cusp of a groundbreaking cure for paralysis through experimental spinal treatments performed on homeless individuals.

As Guy learned more about the organization's true intentions, he found himself at odds with Myrick, who justified his methods as necessary sacrifices in the pursuit of a life-saving breakthrough. Guy, however, refused to condone the exploitation and slaughter of innocent people, labeling Myrick a cold-blooded killer. The latter's demise came unexpectedly, courtesy of rogue FBI Agent Frank Hare.

In the aftermath, Myrick's widow entrusted Guy with the research materials and documentation, revealing her husband's true intentions: "He was trying to do good, but in the wrong way." As Guy pored over the evidence, he became determined to rectify the situation, setting his sights on the very neurology building where he now worked.
