Scott Barnes [John Travolta] is a former ad executive turned social worker in Miami, carrying the weight of a painful past as a recovering alcoholic who stopped drinking after a tragic drunk-driving crash that claimed his son. His work brings him face to face with Tommy [Joey Lawrence], a street kid caught in the clutches of the Youth Incentive Program (YIP). Barnes quickly intuits something is wrong: Tommy buys costly gifts for his mother and sister, and the YIP tattoo on his arm seems to mark him in a way that doesn’t quite fit his story.

Tommy is seized by YIP and forced to churn out crack in vials inside an abandoned building crowded with other children. When Tommy vanishes for days, his sister calls the morgue, where she’s told he’s dead. Barnes goes to identify the corpse, only to discover it’s not Tommy, though the body bears the familiar YIP tattoo. Amid a street shootout he witnesses, a gunman also sporting the YIP tattoo appears, deepening his suspicion that the gang is behind the boy’s disappearance.

Seeking answers, Barnes seeks out Sgt. Falco [Bernie Casey], who lays out the harsh truth about YIP. He then presses Lt. Ortega [Héctor Elizondo], head of the narcotics division, who urges him to stay away from the gang, warning of serious consequences if he pursues them. On the streets, Barnes spots a YIP henchman named James [Ramón Franco] and tails him to a nightclub. There he runs into his former lover Jackie [Marilu Henner], and their reunion is charged with both history and danger. After they sleep together, Barnes learns that Jackie is a lawyer for YIP, and, driven by a desperate determination to save Tommy, asks to be inducted into the organization. After some hesitation, Jackie confirms that YIP is willing to recruit him—a move that puts Barnes closer to the inner workings of the gang than he ever imagined.

Barnes finally meets Carlos [Benjamin Bratt], the sharp-eyed leader of YIP who envisions expanding the operation into suburban neighborhoods. James guides Barnes through the inner circle, eventually leading him to the Madison House, an abandoned factory where YIP members who break rules are killed. Tommy attempts to escape but is dragged back, and Barnes learns of the grim routine from one of Tommy’s friends. The truth becomes personal as Barnes is captured and taken to the Madison House, where he finds Tommy and confronts the escalating danger. He then seeks help from Lt. Ortega, who warns that further interference could result in charges of conspiracy to distribute narcotics.

Carlos’s suspicions harden when one of his henchmen sees Barnes at the police station and reports back that the “investigator” might be FBI. Undeterred, Barnes returns to Jackie’s circle, only to discover that she has been killed by Carlos’s men. He escapes the ensuing pursuit but is soon recaptured and dragged back to the Madison House. In a tense sequence, he’s tossed into an elevator pit filled with alligators, yet he manages to catch himself on wiring and crawl free. With a final confrontation at hand, Barnes seeks out Carlos, and in a brutal fistfight as the police raid closes in, he drives Carlos into the same elevator pit, ending the tyrant’s hold on the operation.

In the aftermath, law enforcement closes the case and arrests the remaining YIP members. Barnes, carrying Tommy away in a police car, steps into a fragile moment of relief, bearing the burden of the past while trying to rebuild what his life and the lives of those around him have become.
