Boz [Travis Hammer] leads a dangerously volatile gang that peddles a powerful, addictive drug called Hype. The Hypers are so hooked that, in a brutal display of power, Boz drops a vial of Hype and Lucy falls from a balcony to her death. Lucy’s younger sister Elizabeth, nicknamed Lizard Sierra McCormick, witnesses the horrific moment and makes a daring escape, stealing Boz’s stash as she flees his reach. The night marks a devastating blow that reverberates through both sides of the conflict and sets the stakes for what comes next.

Elsewhere, Fred Parras [Stephen Lang], a Vietnam War veteran, reconnects with Abe Hawkins [Fred Williamson] as they head to Fred’s bar, a VFW hangout where old comrades gather to relive their glory days. The mood shifts as they reunite with Walter Reed [William Sadler], Lou Clayton [Martin Kove], and Thomas “Z” Zabriski [George Wendt], along with Doug McCarthy [David Patrick Kelly]. They’re there to celebrate Fred’s birthday, but the reunion is interrupted when Shaun Mason [Tom Williamson], a young Army Ranger on leave, joins the group. The atmosphere turns tense when Lizard storms into the bar, triggering a brutal confrontation that culminates in Roadie’s brutal strike, as he hacks off one of Doug’s arms with an ax.

In the chaos that follows, Fred and Z try to reach Z’s truck, only to be intercepted by Boz’s right-hand woman, Gutter, who slices Z’s neck with a machete. Fred survives by stabbing Gutter with the truck keys, then returns to the bar and presses Lizard to tell the truth about the attacks, though she remains silent. Lou discovers the stolen stash hidden in a toilet tank and contemplates making a deal with Boz, but Lizard insists that Boz will never let them live after Roadie’s assault. With little choice, the veterans prepare makeshift weapons from the bar’s furniture and set traps reminiscent of their time in Vietnam, bracing for a night of terror.

The standoff intensifies as Boz’s Hypers, including the massive Tank [Josh Ethier], surge through their barricade in a bloody clash. Faced with overwhelming odds, Lou attempts to broker a deal with Boz to buy time, but Boz refuses and shoots Lou, who dies in the doorway after taking down more of Boz’s men. Fred chases after Lou and is shot in the leg by Boz, spiraling into a despairing slump as Doug succumbs to his wounds and bleeding. Lizard snaps Fred out of it, reminding him that his surviving friends still need him.

Reinvigorated, Fred anchors a last stand as the dawn approaches. The Hypers blow open the barricade and demand the return of the product, but Fred refuses to surrender. In a defiant move, he fires a brick of Hype into the air, sending Boz’s followers into a frenzy, and the veterans begin a ferocious counterattack. Abe, fueled by a dose of Hype, wields his old service machete to cut down pursuing enemies, allowing the others to slip away. Walter commandeers a chainsaw, and with Shaun’s help they race toward their old Gun Truck to escape, though Walter sustains a mortal shoulder wound.

Abe battles Gutter again, ultimately dying when Fred drives a broken flag pole through her body. Lizard is captured by Boz during the chaos, but Fred reloads his shotgun and shoots Boz in the back, then sets the backpack Boz had earlier soaked with alcohol ablaze, enabling Lizard to break free. Walter, barely hanging on, orders Shaun to save themselves and rushes the Gun Truck toward Boz, sacrificing himself as both their vehicle and Boz’s escape car explode, ending Boz’s reign and his gang.

Fred, Shaun, and a wounded Lizard return to the bar, where Abe awaits them, injured but alive enough to share a final moment. They pour shots for their fallen friends, including Abe, who succumbs to his injuries. Fred discovers a block of Hype hidden behind the counter, a stash that Lizard had placed there as a thank-you gesture for all Fred did. The trio drinks in somber celebration, acknowledging the hard-won peace they managed to secure that night, and Shaun quietly wishes Fred a Happy Birthday as the smoke clears and dawn breaks.
