Darla, Darla Hood has just undergone a tonsillectomy, and the group of mischievous kids calls themselves the Third Ward Sunshine Spredders Club. They arrive at the children’s ward with a massive picnic basket full of foods Darla can’t eat right now, planning to devour the whole spread themselves if she refuses. Spanky, George McFarland, leads the charge and assigns Alfalfa, Carl Switzer, to stay outside and guard the basket until they’re certain Darla is still too sick to join the feast inside.

Inside the room, a scheming fellow patient pretends to be sick too and lures Alfalfa into a cheeky exchange: a dime for him to switch clothes and go for a little walk, as if he were the one in charge of the operation. At the same time, a small ally named Gary, Gary Jasgur, has managed to stow away on a gurney being wheeled toward the Tonsil Surgery Wing. In the elevator, Gary cracks open a canister of laughing gas, and the harmless prank spirals into comic chaos as he, Alfalfa, the orderly, and even the elevator operator fall into fits of ridiculous laughter.

The elevator ride becomes a rolling stage for slapstick mischief: Alfalfa, swept up in the gas-induced mirth, drifts through the corridors with a daydreamer’s grin while Spanky and the others scramble to keep up. The mood toggles between frantic pursuit and goofy triumph as the friends try to corral him back toward Darla’s room. In the confusion, Spanky, Porky, Buckwheat, and Percy—Eugene ‘Porky’ Lee, Billie Thomas, and Leonard ‘Percy’ Landy—do their best to rein in the escalating chaos, each adding a distinct flavor to the hospital caper.

When the dust settles enough to reassemble in the room, a still-tipsy Alfalfa hops onto Darla’s bed and lunges for the chandelier, only to tumble down and knock a pitcher of water off the dresser, dousing his head and breaking the spell of the moment. The comic fever gives way to a small moment of relief as the gang quietly settles back, but the situation takes a turn for the inevitable: a nurse arrives bearing Darla’s afternoon ice cream, the one treat she can enjoy.

The nurse’s arrival also brings a tray for the six visiting boys, and the room fills with the sweet scent of ice cream as they dig into the picnic, pairing cold dessert with hamburgers, pickles, hot dogs, and watermelons in a curious, carefree fusion. An animated, lighthearted sequence shows a hot dog writhing against an ice cream scoop, underscoring the kids’ gleeful appetite and the ensuing trouble. The consequence is predictable but harmless: the group encounters stomachaches that land them in the hospital for indigestion, just as Darla is released to go home.

Before they part, Darla and her mother promise to return the next day with “some nice presents,” a gesture that fills Spanky and Alfalfa with a sheepish mix of gratitude and dread. As they’re wheeled away for treatment, they mutter a resigned, comedic plea: “just bring flowers,” a line that lingers as a reminder of the mischief they stirred and the playful courage that bound them together on that bright hospital afternoon.
