Katherine Mortenhoe, Romy Schneider, lives in a near-future world where death from illness has become rare, and her diagnosis of an incurable disease turns her into a national sensation. Journalists swarm, cameras hover, and the line between privacy and fame grows razor-thin as she becomes the focal point of a looming reality spectacle. A powerful television executive, Vincent Ferriman, Vadim Glowna, from the network NTV, offers a life-changing sum of money to film her last days and turn them into a reality show called Death Watch. The deal rests on a troubling foundation: her doctor colludes with the network, secretly recording her diagnosis and handing her painkillers that are anything but soothing.

Katherine agrees to the deal, accepting £300,000 cash upfront and, after handing the money to her husband, slips away in disguise to escape the glare of the city. She drifts toward anonymity, seeking shelter at a night venue for the down-and-out, where she is found by Roddy, a senior NTV cameraman who is not what he seems at first glance. Roddy, Harvey Keitel, has undergone a radical medical modification — an experimental implant of cameras and transmitters behind his eyes that feeds everything he sees back to NTV for the show’s feed.
