After successfully leading a rescue operation for hostages in the bustling city of Mombasa, U.S. Marine Ray Garrison and his wife, Gina, set off for a relaxing holiday in an idyllic town on the Amalfi Coast. However, their peaceful getaway shatters when they are kidnapped by mercenaries under the command of Martin Axe. Axe demands to know how the military discovered the location of the hostages. Despite Ray’s refusal to answer, the situation escalates tragically, leading to Gina’s execution right before Ray’s eyes. Enraged and heartbroken, Ray swears to take vengeance, but Axe brutally kills him too.
In a shocking turn of events, Ray awakens in the high-tech laboratories of Rising Spirit Technologies (RST) in Kuala Lumpur. Here, he meets Dr. Emil Harting, the CEO and lead scientist, who informs Ray that he is the first successful subject of the Bloodshot program. Thanks to cutting-edge Nanite technology, his body is healed and strengthened, replacing his entire bloodstream. Although this grants him incredible powers, Ray must constantly recharge the Nanites or face deterioration and eventual death.
As Ray starts to adjust to his new existence, he is introduced to other patients of RST, including the resilient former Navy Diver KT, who lives with a life-altering injury, Marcus Tibbs, a former Army Ranger with ocular prosthetics, and Jimmy Dalton, an ex-SEAL who has been fitted with new legs after losing his.
Haunted by flashbacks of Gina’s death and the figure of Axe, Ray impulsively decides to seek vengeance. Utilizing his Nanite capabilities, which enable him to hack into various databases, Ray successfully tracks down Axe in Budapest. He confronts him there, eliminating Axe and his bodyguards with intense brutality. Although he sustains several critical injuries during the fight, Ray’s Nanites keep him alive. Before dying, Axe phones a contact, ominously declaring, > “they have finished it, and now it’s here,” suggesting a larger conspiracy at hand.
Afterward, Ray is put back to sleep at RST for a reboot of his Nanite system. However, each time he undergoes this procedure, Dr. Harting alters Ray’s memories to redirect his quest for vengeance towards other targets, disregarding the ethical implications raised by KT. As Harting aims to eliminate anyone who poses a threat to his technology and market exclusivity, Ray awakens once more with fragmented memories, leading him down a violent path to find the person he believes is responsible for Gina’s death.
This cycle of manipulation continues as he tracks down Nick Baris, believing him to be involved in their kidnapping. After slaying Baris, a desperate Wilfred Wigans, who had been trapped in Baris’s web, activates an EMP bomb, freeing Ray from RST’s control. Ray begins to unravel the truth behind his memories, realizing that RST has been using him as a pawn for their own agenda and learning that Gina is, surprisingly, still alive but has moved on with her life.
As chaos unfolds, KT faces her own battle against Harting’s control while attempting to help Wigans escape. Together, they march into a thrilling confrontation with Ray. With the help of his newfound allies, Ray reclaims his autonomy and embarks on a quest to dismantle RST. The confrontation culminates in a fierce battle, leading to the destruction of the RST facility, and ultimately, a showdown with Dr. Harting.
In a desperate attempt to reclaim his lost memories and end the nightmare, Ray confronts Harting, who believes Ray has depleted his strengths. In a bold twist, Ray chooses to sacrifice himself, activating a grenade intended for Harting that results in both of their demises. Miraculously, Ray awakens once again, this time fully aware of his past, with Wigans having upgraded his Nanite technology for self-sufficiency. With KT by their side, they set off towards an uncertain future, seeking redemption and a new beginning.
