Two stories unfold across two interconnected eras, weaving memory, love, and conflict into a single narrative. Set in the late 19th century Russian Empire (1872–1875) and in the early 20th century during the Russo-Japanese War in Manchuria (1904), the film begins thirty years after the tragic death of Anna Karenina on the train tracks, a moment Tolstoy never revisited in his novel.

In 1904, a field hospital convoy is retreating with the Russian army, and the chaos of war frames the action. When the chief doctor and the hospital superintendent go missing, senior resident Sergey Alexeyevich Karenin is forced to take over leadership and steady the crew under pressure. The hospital has been relocated to a semi-abandoned Chinese village, where the wounded are treated and the wounded colonel Alexey Kirillovich Vronsky lies in a fragile state, his condition mirroring the fragility of the larger war-torn world surrounding them.

That night, Sergey visits the ward and asks him to explain why his mother, Anna Karenina, whom the count had known thirty years earlier, threw herself under a train, and whether anything could have changed that day. The aged Vronsky, with a calm gravity, cautions that every person harbors their own truth, and he agrees to tell the grown son of the woman he once loved the story of his love for Anna, a tale that time has already colored with memory and consequence.

As the conversation unfolds, the film traces how a single, forbidden romance shaped generations, weaving the melancholy of a vanished era with the immediacy of war-torn survival. The two timelines—one steeped in the manners and distances of imperial Russia, the other pressed by the immediacy of battlefield medicine and demanded decisions—interlock to reveal how love, pride, and regret can echo across decades. The result is a nuanced meditation on truth, memory, and the ways in which the past continues to inform the present, even when the present is defined by crisis and displacement.
