Kambilikandam Jose, Subair a drug baron in Kochi, travels to meet Pattakkalil Purushothaman “Appichai” Pilla, Vijayaraghavan one of South India’s largest drug dealers, hoping to secure a massive hike in his commission. Appichai arrives with his foster son Sethumadhavan “Sethu”, Saikumar. Appichai calls Sethu, who is on a cruise in Goa. Upon his return to Kochi, Sethu kills Jose with the help of IG Balagopal, Ramu. The murder triggers a strong wave of protests across the state, and the Chief Minister Bhargavan Menon, Janardhanan, orders a detailed investigation to uncover the truth behind the violence and the power network fueling it.

ACP Narendran “Nari”, Mammootty, is placed in charge of the inquiry. Widely known for his aggressive and belligerent approach, Nari begins by questioning Dr Ratnakumari, Urmila Unni, the assistant surgeon who performed the postmortem on Jose.
