Rajaraman, nicknamed “Vasool Raja,” is a local Chennai don who makes a living by extorting money from people with the help of his loyal right-hand man, Vatti. The role is portrayed by Kamal Haasan. To soften his image, he fabricates the existence of a charitable hospital in his father’s name, presenting himself as a man who honors his father’s wish to see him become a doctor, even while his true activities stay hidden beneath a veneer of respectability.

A year into his double life, Raja’s carefully built story begins to crumble when his father, Sriman Venkatraman, an old acquaintance of Dr. Vishwanathan, reappears in the picture. Dr. Vishwanathan, who is played by Prakash Raj, confronts Raja with the revelation of his deceit. In a bid to settle old scores and secure a bridge to respectability, Vishwanathan and Venkatraman hatch a plan to marry Raja to Vishwanathan’s daughter, Janaki Vishwanathan (Paapu), portrayed by Sneha. The arrangement sets the stage for a clash between two generations and two very different visions of legitimacy and honor.

As the truth about Raja’s life spreads, Vishwanathan’s anger grows, and he openly insults Raja’s parents, accusing them of blindness to their son’s true nature. The shock drives Venkatraman and Kasturi, Raja’s mother, to abandon him and move back to their village in defeat and heartbreak. The family rift deepens the tension, leaving Raja’s path to redemption looking precarious and uncertain.

Driven by a stubborn mix of pride and longing for respect, Raja resolves to prove himself by becoming a doctor. He enrolls in medical college, seeking an MBBS degree, and with the help of Vatti and a circle of loyal allies, he secures admission to the SLR Institute of Medical Sciences. Inside the campus, he reconnects with Vishwanathan, who now stands as the college dean and tries to keep a tight rein on the students and the curriculum. Raja’s presence unsettles the conventional order, and he relies on Margabandhu, a faculty member, to maneuver through the system. The first-time appearance of Crazy Mohan as Margabandhu introduces a character who is both a provocateur and a foil to the sterile, impersonal atmosphere of medical education.

Raja’s most controversial innovation is the Kattipudi Vaithiyam—a compassionate approach he learned from his mother, focused on comforting patients through warmth and personal connection rather than cold clinical detachment. Janaki, meanwhile, grows increasingly fond of Raja, and he finds himself more deeply attracted to her as their bond deepens. Vishwanathan tries several times to have Raja expelled, but he is repeatedly outsmarted by the younger man and the loyalty of Raja’s gang.

The tension peaks when Margabandhu’s senile father is admitted to the hospital, and a dramatic carrom match is arranged as a turning point. Raja orchestrates a moment of miraculous revival: Margabandhu’s father awakens and beats the odds, validating Raja’s belief that care and hope can arise in surprising ways. Vishwanathan orders Raja to leave and brings in the police, but student protests surge and block the doorway, forcing him to rethink his position. In a bold test of character, Vishwanathan sets a challenge: Raja must answer every question posed by the doctors in front of the entire university to prove his merit. Raja accepts, and for a time, his performance seems to rise to the occasion, even as a critical case—patient Zakir—deteriorates and dies in his hands.
