Satya, Ram Pothineni a fresh MBBS graduate, moves to Kurnool with his mother Nadhiya and his sister Saranya Pradeep after securing a doctor’s post at a government hospital. From the start, he stumbles upon a dangerous web led by Gururaj “Guru,” a ruthless gangster whose company manufactures poisonous saline and feeds a medical mafia that quietly claims young lives. The city grows tense as three children die under hospital care, yet the police stay inert, cowed by Guru’s reach. Satya files a complaint backed by concrete evidence, and the news travels fast through the hospital and streets alike. Guru learns of the challenge and, in a brutal display of power, attacks Satya at the historic Konda Reddy Fort and hangs him, leaving the young doctor for dead. Miraculously, Satya is saved by his hospital dean, Jayaprakash as Dr. Robert, and the family flees the city to regroup.
Two years pass. Satya returns to Kurnool not as a hopeful doctor but as the city’s new DSP, a position he uses to wage a calculated war against Guru and his men. He hunts for truth, convinced that Guru pressed the trigger on Dr. Robert’s supposed accident, and he presses for answers through a meticulous re-autopsy. In the graveyard, Satya uncovers a chilling twist: a second body lies beneath the one buried. Guru reveals a damning confession: he killed Dr. Robert when the latter aided Satya’s earlier escape. He also claims responsibility for Rahim’s death, shielding himself from exposure by silencing witnesses. The political arena calls Guru to the election stage, but Satya’s persistence pays off when a flash drive containing Guru’s illegal activities surfaces, giving Satya the leverage he needs to move against him legally.
Guru’s wife, Akshara Gowda as Swarna, hatches a final, fatal bid to finish Satya. She schemes to string him up, but Satya’s life takes another turn when his RJ girlfriend, “Whistle” Mahalakshmi Krithi Shetty, steps in. In a tense sequence, Mahalakshmi gets entangled in the same noose and suffers a severe cut to her throat, sending Satya into a hospital room full of fear and resolve. Satya’s mother, steadfast and hopeful, urges him to persevere, and together they navigate the fallout of Swarna’s plot and Guru’s return from prison.
