When Bobby Saint and Mickey Duka meet with European arms dealer Otto Krieg at the Tampa, Florida seaport, their encounter takes a deadly turn as the FBI intervenes. In the chaos, Bobby meets his end, leaving Duka in jail. However, the man they believed to be “Krieg,” who supposedly perished during the shootout, is in fact Frank Castle, an undercover FBI agent and former U.S. Army Delta Force operator portrayed by Thomas Jane. Following this incident, Frank retires from the FBI and heads to his family’s reunion at his father’s ocean-side home in Aguadilla Bay, Puerto Rico.
Infuriated over the loss of his son, Tampa crime lord Howard Saint seeks revenge with the assistance of his right-hand man, Quentin Glass, who secretly bribes the FBI for intel on Frank. When Howard discovers Frank’s identity, he plots to eliminate him and orders the assassination of Frank’s entire family to “settle the score.”
During the reunion, tragedy strikes as gunmen, including Glass and John Saint—the identical twin of Bobby—massacre Frank’s relatives. In a desperate fight for survival, Frank and his father, played by Roy Scheider, manage to fend off some attackers, but tragedy leaves Frank’s father dead and his wife and son perish in a gruesome attack executed by Saint himself. Left for dead after suffering a gunshot wound and an explosive incident orchestrated by Glass, Frank is saved by a local fisherman named Candelaria.
Once he regains his strength, Frank heads back to Tampa, seeking solace in an old tenement shared by three outcasts: Dave, Bumpo, and Joan. In a bid for information, he captures Mickey Duka, who soon becomes his ally after discovering a mutual loathing for the Saints. Frank embarks on a relentless quest for vengeance against both corrupt police officers and FBI agents who have conspired to bury the investigation into his family’s demise. He disrupts Howard’s money-laundering operations and severs ties with the Cuban Toro Brothers, while tracking Livia, Howard’s wife, and Glass, whom he learns harbors a secret sexual orientation.
Realizing that Frank is alive, Howard dispatches assassins, the first being guitar player Harry Heck. Frank eliminates him with brutal efficiency, shooting a ballistic knife into his throat. The second assassin, “the Russian,” presents a formidable challenge, overpowering Frank in a fierce battle. However, Frank manages to gain the upper hand by tossing boiling oil at him, allowing him to throw the hulking brute down a staircase, snapping his neck.
When Howard’s men, led by Glass and John, come searching for Frank, the trio of Dave, Joan, and Bumpo defiantly protect Frank, even amidst threats of violence. After a brief hospital trip for Dave, Frank prepares for the final confrontation against Howard and his gang.
With Mickey’s help, Frank manipulates Howard into believing that Glass and Livia are engaged in an affair. Blind to their real relationship, Howard viciously murders them, his desperation driven by jealousy as he offers a bounty to anyone who can bring him Frank’s head.
In a dramatic showdown at Howard’s nightclub, Saints and Sinners, Frank takes on the criminal element, dispatching numerous henchmen and culminating in a brutal demise for John Saint. Frank’s final confrontation leads him to the parking lot, where he delivers a devastating blow to Howard. Tying him to a moving car, Frank reveals the truth about Glass and Livia before triggering a series of hidden bombs that obliterate Howard in a spectacular fashion, leaving behind a trail of destruction that resembles the Punisher’s iconic skull.
As night falls, Frank faces a moment of introspection in his apartment, contemplating taking his own life. However, encouraged by a vision of his wife, he resolves to continue his crusade against wrongdoers. Before setting off on the next chapter of his mission, he leaves a portion of Howard’s ill-gotten gains for Bumpo, Joan, and Dave. Standing on the Sunshine Skyway Bridge, he declares, > “Those who do evil to others—the killers, the sexual predators, psychos, sadists—will come to know me well. Frank Castle is dead.
