Cha Tae-sik is a quiet pawnshop owner whose only real friend is So-mi, a young girl from the same neighborhood. So-mi’s mother, Hyo-jeong, is a go-go dancer and opium addict who becomes tangled in a dangerous drug underworld when her lover orchestrates a theft. She hides a large pack of opium in a camera bag and pawns it to Tae-sik for safekeeping. Drug lord Oh Myung-gyu assigns his brothers, Man-seok and Jong-seok, to recover the drugs. Jong-seok locates Hyo-jeong and brutalizes her in front of So-mi until she reveals the location of the stash, escalating the threat around Tae-sik’s quiet life.

Du-chi and Bear go to the pawnshop, and Tae-sik proves to be a formidable force as he overpowers them, all while So-mi is held in dangerous limbo. Jong-seok forces Tae-sik to deliver the opium to Myung-gyu, while Man-seok, growing weary of serving his brother, leaks word to the police — though Myung-gyu manages to slip away from arrest. After discovering Hyo-jeong’s body with her organs harvested in the back of his car, Tae-sik is arrested, yet he escapes the police station, stunning the authorities with his dramatic combat skills. Investigators soon learn that Tae-sik was a former covert operative for South Korean Army Intelligence, who retired after his pregnant wife was killed by an assassin.

The brothers close in, hunting down Hyo-jeong’s lover and killing him, while their bodyguard Ramrowan extinguishes Myung-gyu’s operation, enabling the brothers to tighten their grip on the drug trade. Following a lead from a burner phone Jong-seok provided, Tae-sik traces Du-chi to a nightclub, where Ramrowan unexpectedly intervenes, shooting at the trio, killing Du-chi and wounding Tae-sik. Tae-sik escapes and reconnects with his former partner, who tends to his wounds and helps him obtain a gun. Returning to the city, Tae-sik uncovers a drug manufacturing plant and, in a fierce confrontation, destroys it, killing Jong-seok and freeing the enslaved child workers in the process.

Tae-sik then faces Man-seok’s gang at his condominium. He confronts Man-seok, who reveals that So-mi’s eyes were harvested and shows a container containing two preserved eyeballs. Enraged, Tae-sik brutalizes the gang members, including Ramrowan and Man-seok. As grief weighs heavily on him, Tae-sik contemplates suicide, but So-mi—clever and unscathed by the ordeal—finds him. Ramrowan, who had spared her life out of pity, had kept her safe, and the container’s eyes belong to the gangsters’ surgeon. The police allow Tae-sik and So-mi to ride away together after Tae-sik is arrested.
