In August 2005, we meet Terrence McDonagh, a New Orleans Police Sergeant portrayed by Nicolas Cage. While he is sorting through a locker in the aftermath of Hurricane Katrina, he stumbles upon a prisoner on the verge of drowning in his cell. Displaying a cruel sense of humor, he mocks the man before bravely diving in to rescue him. This act of heroism earns him a promotion to Lieutenant along with a medal; however, it comes at the cost of a severe back injury that necessitates lifelong Vicodin use.
Fast forward six months, and Lt. McDonagh finds himself ensnared in a web of addiction, not just to the painkillers but also to cocaine and marijuana. He has managed to manipulate an employee in the police property room to smuggle drugs from the evidence locker. His relationship with his girlfriend, Frankie Donnenfeld, played by Eva Mendes, is equally destructive as they share a penchant for cocaine. Their toxic lifestyle has alienated McDonagh from his family, particularly his father, a recovering alcoholic, and his equally troubled stepmother.
As the plot unfolds, Lt. McDonagh uses his badge to intimidate and exploit vulnerable individuals for his drug supply. When he discovers Frankie has been assaulted by a client, his violent protective instinct kicks in, and he threatens the aggressor with dire consequences.
Assigned to probe a grim murder scene where five illegal Senegalese immigrants were executed possibly over a drug deal, McDonagh’s investigation leads to a delivery boy who witnessed the crime. With the help of this witness, they uncover the involvement of a local gang led by Big Fate. The gang members are swiftly arrested, yet their witness mysteriously vanishes.
In a desperate attempt to locate the missing witness, McDonagh confronts an elderly woman at a nursing home, inadvertently causing chaos. The woman reveals that the witness fled to England to escape Big Fate’s murderous reach. Complicating matters further, McDonagh is embroiled in a dispute with his bookie over a $5,000 debt. This leads him to form an illicit partnership with Big Fate, intending to gather information while attempting to clear his financial woes. Their agreement culminates in violence when McDonagh pays his bookie, leading to the demise of the figurehead who once threatened them.
