Sanjay Rajkot, the final heir of the Rajkot dynasty of Gujarat and widely known as “Sikandar,” is married to Saisri, a painter who is affectionately called Rani Sahiba. During a routine flight, Arjun, the son of Rakesh Pradhan, confronts Monica, a former pornographic actress who now has a husband and a son. He menaces her with the threat of exposing past videos to her child unless she concedes to his demand for sex. On the same flight, Sanjay intervenes, stepping into a tense confrontation with Arjun and his bouncers. A fierce fistfight ensues, and Sanjay ultimately compels Arjun to apologize to Monica. The incident marks the uneasy balance between power, reputation, and personal history that threads through the rest of the story.

The following day, Rakesh hires Inspector Prakash to arrest Sanjay for assaulting his son, sealing the deal with a personal bribe. Prakash arrives at the Rajkot mansion only to learn that Sanjay has already headed to the police station. There, he finds Sanjay surrounded by his senior officers, who sheepishly order him to let the matter drop. The case tests loyalties within the system and underscores how quickly justice can bend under political pressure, while Saisri begins to feel increasingly distant from her husband—often forgetting their wedding anniversary and her birthday.

Meanwhile, Saisri discovers she is pregnant. At almost the same moment, a crisis unfolds at a quarry owned by Sanjay’s business circle when the quarry manager sells 40 kilos of explosives intended for a dangerous site. Sanjay races to avert a disaster, but unknowingly, Saisri follows him to the location and is caught in a bomb blast. She sustains grave injuries and dies before she can reveal the news of her pregnancy to Sanjay. Before she dies, she signs up for organ donation, and her lungs, eyes, and heart are transplanted into patients in Mumbai.

Haunted by the loss, Sanjay travels to Mumbai to locate the organ recipients. He learns that his wife’s lungs went to a child named Kamar, an orphan from Dharavi who struggles with a severe lung disease caused by intense pollution. The pollution is driven by Virat Bakshi, a ruthless businessman who has bought up land to push residents off and uses a nearby dumping ground to worsen living conditions. Sanjay also discovers that Vaidehi Ranga received Saisri’s eyes, and Nisha received her heart, sparking a personal mission to protect and connect with the lives saved by his wife’s donations.
