Sanjay Rajkot, the final heir of the Rajkot dynasty of Gujarat and widely known as “Sikandar,” is married to Saisri, a painter who is affectionately called Rani Sahiba. During a routine flight, Arjun, the son of Rakesh Pradhan, confronts Monica, a former pornographic actress who now has a husband and a son. He menaces her with the threat of exposing past videos to her child unless she concedes to his demand for sex. On the same flight, Sanjay intervenes, stepping into a tense confrontation with Arjun and his bouncers. A fierce fistfight ensues, and Sanjay ultimately compels Arjun to apologize to Monica. The incident marks the uneasy balance between power, reputation, and personal history that threads through the rest of the story.

The following day, Rakesh hires Inspector Prakash to arrest Sanjay for assaulting his son, sealing the deal with a personal bribe. Prakash arrives at the Rajkot mansion only to learn that Sanjay has already headed to the police station. There, he finds Sanjay surrounded by his senior officers, who sheepishly order him to let the matter drop. The case tests loyalties within the system and underscores how quickly justice can bend under political pressure, while Saisri begins to feel increasingly distant from her husband—often forgetting their wedding anniversary and her birthday.

Meanwhile, Saisri discovers she is pregnant. At almost the same moment, a crisis unfolds at a quarry owned by Sanjay’s business circle when the quarry manager sells 40 kilos of explosives intended for a dangerous site. Sanjay races to avert a disaster, but unknowingly, Saisri follows him to the location and is caught in a bomb blast. She sustains grave injuries and dies before she can reveal the news of her pregnancy to Sanjay. Before she dies, she signs up for organ donation, and her lungs, eyes, and heart are transplanted into patients in Mumbai.

Haunted by the loss, Sanjay travels to Mumbai to locate the organ recipients. He learns that his wife’s lungs went to a child named Kamar, an orphan from Dharavi who struggles with a severe lung disease caused by intense pollution. The pollution is driven by Virat Bakshi, a ruthless businessman who has bought up land to push residents off and uses a nearby dumping ground to worsen living conditions. Sanjay also discovers that Vaidehi Ranga received Saisri’s eyes, and Nisha received her heart, sparking a personal mission to protect and connect with the lives saved by his wife’s donations.

As he unravels each recipient’s story, Sanjay meets Kamar, who shows how pollution has choked his breath and future. He then encounters Vaidehi, who runs a small vada shop but has seen orders dry up, her household dynamics strained by a father-in-law who believes a woman should not work. Finally, he learns that Nisha, hospitalized because her donated heart began failing, was born with a congenital condition that grew worse after her naive ex-boyfriend, Kapil, informed her that he was marrying someone else, causing her emotional distress. To help her, Sanjay arranges for a Dubai-based doctor to guide the local team via video call, stabilizing her condition. The connection with Nisha fuels Sanjay’s determination to protect the three lives tied to his wife’s organs, and he confronts Kapil to ease Nisha’s burden.

As Arjun reappears at a rally where Sanjay speaks, a suspenseful cat-and-mouse chase erupts. In the confrontation’s climax, Arjun accidentally kills himself by dousing his own car with Molotov cocktails. With Sanjay prepared to move on, a chilling phone call from Rakesh interrupts the tenuous peace: he swears revenge and vows to destroy all three organ recipients’ lives. Yet Sanjay remains steadfast, vowing to protect them and choosing to stay in Mumbai to oversee their safety and future.

In the months that follow, Sanjay quietly helps the three donors and their families flourish—Kamar’s health improves enough to let him breathe easier, Vaidehi’s business finally takes shape with her in charge, and Nisha builds a happy life with her loved ones. But the victory is tempered when Rakesh orchestrates a fake terrorist bomb attack that casts doubt on Sanjay’s integrity. His friends rally to clear the misunderstanding, and when Rakesh sends henchmen to attack the hospital where Kamar is hospitalized, Sanjay fights them off with determination and skill. The confrontation culminates in Sanjay breaking into Rakesh’s mansion and delivering a decisive beating.

Ultimately, Sanjay surrenders to the police, yet the city’s people fill the streets in protest, demanding his release. The public outcry succeeds, Sanjay is freed, and his adoption of the title Sikandar is solidified in the memory of Mumbai’s residents as a man who stood up for life and community—even at great personal cost.
