Jack (Jesse Metcalfe) goes undercover as a new patient to save his sister Lily (Kiele Sanchez), who has been involuntarily institutionalized for reasons unknown. Inside, they discover that a mad doctor at the asylum, Dr. Gianetti (Peter Stormare), has been testing an experimental compound, orphium, on the patients, a drug that seems to awaken a predatory hunger and turn some individuals into flesh-eating zombies. Loomis (Kurt Caceres), another patient, spreads the infection. The siblings join forces with a terminally paranoid man, Dave (Kevin Sussman), and a compassionate nurse, Nancy (Olivia Munn), in a desperate bid to escape.
As chaos erupts, they are attacked by most of the prisoners and staff, who kill Nancy. On the 4th floor, Gianetti confronts them; he attempts to grab Jack, but Loomis ultimately infects Gianetti. The two siblings fight to survive as the asylum devolves into turmoil, with danger lurking around every corner.
At the film’s end, the siblings flag down a pair of officers, Mark Kelly and Sharon Schaffer in their police car, and plead for help. Yet when the officers arrive to investigate, they inadvertently let the patients escape into the outside world, a chilling misstep that leaves the city oblivious to the danger spreading from the hillside asylum. The final shot sweeps across the city below, underscoring a precarious new reality beyond the gates.
The secondary plot threads through an affair between Charles (Evan Parke) and Heather (Lisa Arturo), adding a human shimmer of romance and tension amid the horror, as their relationship unfolds against the backdrop of the unfolding crisis inside the asylum.
