Jamie Morgan is a lonely, conflicted photographer whose life is marked by a large heart‑shaped birthmark that covers nearly half of his face. At 25, his years of alienation and bullying have left him socially adrift and still a virgin, despite a big heart and a stubborn longing for connection. In the photo studio he shares with his brother and nephew Lee, he crosses paths with an aspiring model named Tia, and a fragile spark of possibility flickers into view.

As Jamie develops a shoot, he notices a disturbing presence watching from a window in a nearby house. Curious and unsettled, he follows a suspicious man to a circle of hooded vandals around a campfire. Their shrieks cut through the night, and one shriek in particular reveals a demonic, lizard‑like face beneath the hood, with teeth like sharp pins. Over time, Jamie becomes aware of a string of brutal murders in the neighborhood, all carried out with Molotov cocktails. A witness, a small Asian girl seen on a TV report, asserts that the attackers weren’t masked—those demonic visages felt real. When Jamie’s neighbor and new friend A.J. turns up with a wound that looks claw‑torn and later turns into a pattern of dismemberment nearby, the horror grows closer to home. A brutal attack on Jamie and his mother, in which his mother is immolated and he is left for dead, shatters any sense of safety and ordinary life.

While recovering in a hospital, Jamie experiences a dream‑like visitation from a man who claims to be waiting for him. A chilling phone call later guides him to the apartment of Papa B and his assistant Belle, who bears a striking resemblance to the little Asian girl from the TV report. Papa B offers a Faustian bargain: in exchange for Jamie performing chaotic acts of vandalism, his birthmark will be removed. The admission that he was responsible for his mother’s death as a way to bring Jamie to him makes the proposition feel even darker, but Jamie accepts and shakes hands, agreeing to an unimaginable path. He is told to immolate himself with a Molotov as part of a rebirth, and miraculously survives, peeling away the burned skin to reveal flawless new flesh.

At first, life appears brighter: a chance encounter with Tia helps Jamie blossom into a more confident man, and he spends a day in the park—the place where his late father first taught him to use a camera. Yet the dreamlike pact sours when the Weapons Man arrives; Papa B reneges on the bargain and Jamie learns he must kill by ripping the heart from a living victim. The grip of fear returns, but hope flickers again when he and Tia become lovers. Belle also comes into Jamie’s life, moving in after Papa B’s brutal beating, and she begins calling him “Dad,” offering pragmatic guidance about the grim reality he must face to stay alive—and to protect Belle as well. Papa B’s rage at Belle’s apparent betrayal drives him to demand that Jamie kill, with the next victim being Tia herself.

As tensions mount, Tia confesses a plan to hide in Jamie’s work safe, and the truth surfaces: Tia and Lee had intended to steal Jamie’s late mother’s jewelry. Tia’s feelings deepened beyond the initial scheme, and she falls in love with Jamie. In the ensuing struggle, Tia is accidentally shot and killed, and Lee is severely wounded by She, a gang leader who wears a metallic claw in place of a severed hand. Jamie is forced to confront She, kills him, and notices that a claw‑shaped mark on a wall eerily mirrors A.J.’s wound. He begs Belle to flee, and she vanishes from sight as a chilling sense of abandonment settles in.

A startling return to self‑awareness follows: Jamie looks into a mirror and sees that his birthmark has come back, revealing that it may have been present all along, even when he was pursuing Tia.
