Billy Breedlove, Beau Bridges, is an orderly at a Texas psychiatric hospital who becomes drawn to two powerful influences: a blonde diner waitress named Jimmie Jean Jackson, Elizabeth Taylor, and Hammersmith, Richard Burton, a dangerous patient strapped in a straitjacket within a locked cell. The allure of these two figures pulls him toward a path he never anticipated, blurring the lines between loyalty, ambition, and danger.

Hammersmith promises Billy a new life of fame and fortune if he can win his freedom. Billy agrees to the jailbreak, provided that Jimmie Jean Jackson can accompany them, and the trio slips away into a world where crime becomes a means to expand Billy’s status. From there, Hammersmith murders people and steals property as explicit shortcuts to lift Billy up the social ladder. What follows is a startling ascent: Billy becomes the owner of a topless bar, the head of a pharmaceutical company, an oil tycoon, the financier of political campaigns, and a roving ambassador-at-large for the United States. Each new title seems to promise validation and wealth, yet each step further from the hospital’s quiet routine reveals a deeper hollowness.

As the climb continues, Billy begins to loathe Jimmie Jean Jackson even as Hammersmith keeps an uneasy, almost possessive interest in her. He grants her a disturbing wish that she should become a mother, a moment that unsettles Billy and reveals the true cost of their shared ambitions. In a calculated manipulation, Hammersmith arranges for Billy to suffer a disabling water-skiing accident and then persuades him to end his own life, a finale that underscores how easily power can corrupt and destroy. The hospital’s head finally locates Hammersmith and returns him to confinement, but the threat of his cunning lingers, as he resumes whispering promises of fame and fortune to another orderly, leaving a trail of wreckage in the wake of his influence.
