FBI agent Jake Malloy, Sylvester Stallone is on the trail of a serial killer who targets police officers in Seattle. When the killer phones Malloy from his own home and taunts him about a prior investigation into a string of prostitute murders, the caller makes the danger personal: as revenge, Mary, Dina Meyer, Malloy’s girlfriend, is killed. This brutal move sets Malloy on a relentless pursuit that quickly turns his life upside down.
Three months pass and Malloy has spiraled into alcoholism. After a desperate suicide attempt, his best friend and supervising officer, Agent Chuck Hendricks, Charles S. Dutton, arranges for Malloy to enter a rehabilitation program for law enforcement officers, run by Dr. John “Doc” Mitchell, Kris Kristofferson. Hendricks stays in Wyoming to monitor Malloy and keep him from slipping again, while the clinic’s snowbound isolation creates a tense, claustrophobic mood that sticks with him long after he leaves.
Inside the rehab center, Malloy meets a cast of recovering officers with their own scars. Peter Noah, Jeffrey Wright, is an arrogant ex-SWAT officer who carries himself like he’s still in command; Jaworski, Sean Patrick Flanery, is a narcotics cop who has battled despair; Jenny Munroe, Polly Walker, serves as a kind nurse who offers genuine care; and McKenzie, Robert Prosky, an elder RCMP veteran who witnessed a partner’s murder. There are other staff members—Doc’s assistant and mechanic Hank, Tom Berenger, and the gentle resident psychiatrist—who help anchor the group in their shared struggle.
A blizzard blankets the facility, severing all outside communication and trapping everyone inside. Jenny discovers the body of Connor, a troubled patient who apparently killed himself, but she suspects Connor would have asked for help if he’d truly needed it. The next morning, Hank finds another apparent suicide, and Malloy grows convinced there’s a killer among them. Doc retaliates by locking up the surviving patients as he reviews their files, while Jenny warns that Jack Bennett, an orderly and former patient, is missing. When an axe-wielding intruder kills Doc, distrust snakes through the clinic: the residents fear Jack, even as Malloy dissolves the barrier between patient and suspect and returns the officers’ sidearms.
Hank, Manny, and Gilbert volunteer to drive through the storm to reach outside help, hoping to break the siege. A crash in the ice turns the escape into a nightmare: Manny is killed, Gilbert escapes, and Malloy rushes back to the clinic with Jenny. Malloy escapes, retrieves the missing badges—symbols of the protection the officers thought they were serving—and uses them to help identify Slater’s ruse. Although Malloy has cleared his own name, Slater’s rampage continues.
The action moves into the tunnels beneath the facility. Hank and Noah assist in collecting logs to restore heat, unaware they’re walking into a trap. Slater lures Malloy into danger after another tense exchange, while Jenny and the others are left to guard the exits. Outside, Hendricks and the fishing shop owner locate Gilbert alive and bring him back to the clinic, where Hendricks follows Jenny’s footprints to safety. Jenny flees into a nearby quonset hut, while Malloy orders her to stay put. Slater corners Hendricks and Jenny, but Malloy arrives just in time to intervene. A brutal confrontation ensues, and Malloy defeats Slater with a decisive strike, impaling him on the spikes of a snow machine.
Jenny regains consciousness and helps Hendricks walk toward the clinic as Malloy makes one final, symbolic gesture.
