As Steve Rogers (Chris Evans) navigates his post-Battle of New York life in Washington D.C., working for S.H.I.E.L.D. alongside Agent Natasha Romanoff (Scarlett Johansson) and the elite counter-terrorism team, S.T.R.I.K.E., led by the ruthless Brock Rumlow, he finds himself immersed in a web of espionage and deception. During a daring mission to rescue hostages from a pirate-infested S.H.I.E.L.D. vessel, Rogers discovers Romanoff's true intentions: to extract classified data from the ship's computers.
Upon returning to the Triskelion, S.H.I.E.L.D.'s headquarters, Rogers confronts Director Nick Fury, only to be briefed on Project Insight - a trio of Helicarriers linked to spy satellites designed to eliminate threats proactively. As Romanoff's encrypted files remain inaccessible, Fury becomes increasingly suspicious about the true nature of Insight and orders senior S.H.I.E.L.D. official Alexander Pierce to delay the project. En route to meet with Maria Hill, Fury is ambushed by a team of assassins led by the enigmatic Winter Soldier. Escaping to Rogers' apartment, Fury warns his protégé that S.H.I.E.L.D. has been compromised, but not before he's gravely wounded and hands over a flash drive containing the ship's data.
In a shocking turn of events, Fury is pronounced dead during surgery, while Hill recovers his body. Pierce summons Rogers to the Triskelion, only to withhold Fury's information and brand him a fugitive when he refuses to cooperate. With S.T.R.I.K.E. hot on his heels, Rogers meets with Romanoff, and together they decipher the secrets contained within the flash drive. Their investigation leads them to a hidden S.H.I.E.L.D. bunker in New Jersey, where they activate a supercomputer containing the preserved consciousness of Arnim Zola. The cunning villain reveals that after being captured by Rogers during World War II, he was recruited by S.H.I.E.L.D. and secretly reformed Hydra within its ranks, using the Winter Soldier as their primary assassin to sow global chaos.
As Rogers and Romanoff narrowly escape death when a S.H.I.E.L.D. missile destroys the bunker, they realize that Pierce is none other than Hydra's leader within S.H.I.E.L.D.'s ranks. The pair enlists the help of Sam Wilson (Anthony Mackie), a former USAF pararescueman and VA employee who had befriended Rogers, and together they acquire the powerful "Falcon" wingpack used by Wilson during his time in the Air Force. With their bond strengthened and their determination renewed, Rogers and Romanoff are ready to take on the treacherous world of espionage and Hydra's sinister plans head-on.
As the stakes escalate, Jasper Sitwell (character), a Hydra mole masquerading as an S.H.I.E.L.D. agent, is apprehended and forced to reveal Zola's sinister plan: a data mining algorithm designed to pinpoint individuals who pose a future threat to Hydra's nefarious endeavors. The Insight Helicarriers, equipped with satellite guns, prepare to sweep the globe, eliminating any perceived threats to their organization. Sitwell's life is cut short in a brutal ambush orchestrated by the Winter Soldier, whose true identity as Bucky Barnes (character) is finally revealed to Steve Rogers (character), his former best friend and long-lost ally. The revelation leaves Rogers reeling as he comes to terms with the fact that Barnes had survived Zola's experiments and has been repeatedly brainwashed and cryogenically frozen to carry out Hydra's missions.
Meanwhile, Nick Fury (character) orchestrates a daring plan to sabotage the Helicarriers from within. He manages to extract Steve Rogers, Natasha Romanoff (character), and Sam Wilson (character) to a safehouse, where they concoct a strategy to undermine Hydra's plans. Hill successfully extracts classified information from S.H.I.E.L.D.'s database, which Romanoff uses to expose Hydra's true intentions to the world.
As the World Security Council members converge on the Triskelion to witness the launch of the Helicarriers, Rogers takes advantage of the chaos to broadcast Hydra's sinister plot to everyone in attendance. Romanoff, disguised as a Council member, neutralizes Alexander Pierce (character), the mastermind behind Hydra's nefarious plans. Fury then forces Pierce to unlock S.H.I.E.L.D.'s database, allowing Romanoff to leak classified information and expose Hydra's treachery.
Following a tense confrontation, Fury kills Pierce, marking the end of his sinister involvement. Rogers and Wilson launch a daring raid on two Helicarriers, replacing their controller chips with ease. However, things take a dramatic turn when Barnes destroys Wilson's suit and confronts Rogers on the third Helicarrier. Despite the intense battle, Rogers manages to outmaneuver Barnes and replace the final chip, allowing Hill to regain control of the vessels. The two Helicarriers then engage in a fierce dogfight, ultimately destroying each other.
As the dust settles, Rogers refuses to fight his former friend, opting instead to reach out to Barnes and bring him back from the brink of madness. However, as the ship collides with the Triskelion, Rogers finds himself plummeting into the Potomac River. Barnes rescues the unconscious Rogers and disappears into the woods, leaving behind a trail of mystery.
In the aftermath, Romanoff appears before a Senate subcommittee to defend her actions and those of Steve Rogers, while Fury secretly heads to Europe to track down Hydra's remaining cells. Meanwhile, Rogers and Wilson decide to embark on a perilous journey to find Barnes, who has seemingly vanished into thin air. As for Rumlow (character), a former Hydra agent, he finds himself hospitalized following the destruction of the Triskelion.
In a mid-credits scene, Baron Wolfgang von Strucker (character) and his scientists examine an energy-filled scepter at a Hydra lab, alongside two test subjects: one possessing superhuman speed, the other telekinetic powers. In a post-credits scene, Barnes pays a somber visit to his own memorial at the Smithsonian Institution, leaving viewers wondering about the true extent of his redemption.
