Max, a foster child who already has a shadowed past—a decade after his older foster brother died by suicide—has grown into a graphic artist known for spray-painted works, and his latest act of vandalism lands him in trouble. To avoid a criminal record, his foster father arranges a community-service stint that lands him in a retirement home where he takes on the role of superintendent. He’s warned not to set foot on the mysterious fourth floor, but curiosity quickly nudges him toward the unseen.

In the quiet corridors of the home, Max befriends a handful of residents, including Ellie and Ethan, and discovers a pattern of odd, unsettling events. Nighttime intrusions blur the line between reality and paranoia; residents slip into his room, unusual bleeding appears out of nowhere, and some carry disturbing psychological quirks that hint at a deeper, hidden malaise festering within the building. Gretchen becomes one of the more notable faces he encounters as he pieces together the strange mood of the place, and the eerie atmosphere thickens with every late-night encounter.

Driven by a mix of duty and concern, Max ventures to the fourth floor, where the atmosphere thickens into something otherworldly. The residents here are mostly lethargic and show extreme bodily harm and severe psychoses. An assault by a resident during one of his visits serves as a brutal reminder that he’s not welcome there, and he’s told in no uncertain terms to stay away.

This death becomes a catalyst for Max’s private investigation into the home, peeling back the layers of normalcy to reveal a network of secrets that the staff seems keen to shield.

Back at his foster parents’ house, two younger foster children usher him toward a private room hidden within the residence, a discovery that comes after his foster mother reveals she had known the friendly resident. The revelation hits hard: the seemingly ordinary resident on the fourth floor is revealed to be Max’s own brother, whose suicide had been staged as part of a larger conspiracy.

The truth shatters the surface calm of the retirement home. Max is recaptured, and the staff explain a dark ritual at the heart of the operation: a system that extracts fluid from the eyes of foster children to extend the lifespans of the occupants. The impaled resident’s death is recast as part of the broader pattern of exploitation, a murder carried out to maintain the cycle.

As the plot intensifies, Max is placed back on the fourth floor, becoming a key participant in the disturbing harvest. His brother orchestrates the process, drawing out the eye fluid from the remaining victims—and from himself—before administering it to Max. The sudden influx of fluid grants Max a startling stamina and resilience, enough to confront and dismantle the cult at the heart of the home. In the aftermath, the remaining victims, having reclaimed some of their strength, join in a final push to sever the bond between life extension and exploitation.
