Adithya Varma is the youngest son of wealthy business tycoon Vasudevan Varma, and he is pursuing an MBBS degree in Mangalore, India. Despite his brilliance, he wrestles with severe anger management issues that draw the dean’s ire and earn him a reputation among juniors as a college bully. After a heated brawl with his friend Tarun against an opposing team during an inter-college football match, the dean gives Adithya an ultimatum: apologize or leave the college. He initially opts to depart, but his stance shifts when he meets introverted first-year student Meera Shetty, and love at first sight changes the course of his life.
Together with his friend Parthi, Adithya publicly proclaims that he is in love with Meera and asserts that she is exclusively his. Meera, who is initially wary, slowly adjusts to Adithya’s overbearing personality and eventually returns his feelings, leading to a deepening, intimate bond. After earning his MBBS, Adithya travels to Mussoorie to pursue a master’s in orthopaedic surgery, and over three years their relationship grows stronger. Months later, Adithya visits Meera’s home, where her father witnesses them kissing and expels him from the house.
Meera’s father opposes the match, in part due to Adithya’s brash behavior and the couple’s different castes. Adithya demands a decision from Meera within six hours, threatening to end the relationship otherwise. In the aftermath, Meera’s parents seize her phone, cutting off contact with Adithya. When she finally manages to visit him, he is drunk, injects morphine into himself, and remains unconscious for two days. Meera is then forcibly married to someone from her caste. Adhi learns of the marriage from Parthi and goes to her house to protest, but he is assaulted and arrested for causing a disturbance. With Parthi’s help, Adhi finds a rented apartment and joins a private hospital as a surgeon. To cope with the pain, he slides into self-destructive habits—drug use, aimless flings, a dog he names after Meera, and heavy drinking. Despite these struggles, he becomes a highly skilled surgeon and a respected yet fearsome figure at the hospital, largely due to his impressive surgery count. His friends Parthi and Tarun worry about the toll his behavior is taking.
