In a brutal confrontation, Deva slits Deepthi’s throat and fatally stabs Satya, and as the others flee, they set the bus ablaze, leaving Satya near death and Deepthi lost to violence.
Dr. Sailaja Jayasudha saves Satya by transplanting a new face, giving him a second life under a new look. Ten months pass, and Satya awakens from a coma with a completely different appearance and a new identity, Ram. Returning to Visakhapatnam, he reconnects with a local model named Sruthi, who is now in Veeru’s orbit of desire, and, using his altered face, he lures Deva to a dangerous confrontation in an under‑constructed apartment where he kills him. A photograph of Deva falls into the hands of ACP Ashok Varma, marking it as case file number 1, while Ram files a missing-person report to the police about Deepthi.
As Sravan Sravan Raghavendra grows suspicious of Ram’s unusual presence, he tails him to a mall, where Satya—now Ram—ends him. During the investigation into Sravan’s death, Ram appears to Varma as an eyewitness and, when asked to describe the killer, he unconsciously re-creates his former face. Ram then schemes to win over Sruthi, steering Ajay toward a dangerous fixation with her.
Ram sends compromising pictures of Ajay and Sruthi to Veeru, and he engineers Ajay’s belief that killing Veeru will grant him freedom and the chance to take his brother’s place. Yet Veeru’s henchmen strike first, killing Ajay, and Veeru himself is slain by Ram, who reveals his true identity and apologizes to Sruthi for having used her before leaving Visakhapatnam behind. On the road, a mysterious attacker named Raja ambushes Satya, but is eventually killed, deepening the mystery surrounding Ram’s new face.

Charan’s body is sent to Sailaja, who, after weighing Satya’s will to live, decides to transplant Charan’s face onto Satya rather than perform a simple procedure. Before Satya can tell him, Satya leaves the hospital.
Present day, Sailaja’s decision sets Satya on a path of vengeance: he adopts Charan’s identity to rally the local community, turning the tables on Dharma. He meets Sharath at a political gathering, leveraging the shock of Charan’s apparent return to push Sharath into killing Dharma, after which Dharma’s supporters track Dharma to his home, where Sharath is killed in the ensuing struggle and Dharma is finally slain by Satya.
