Sarah is found after escaping the cave; amnesia and hospital check

Two days after the last film, a traumatized Sarah staggers out of the cave, bloodied and with amnesia. She is rushed to a hospital, where doctors note injuries and memory gaps. The staff discover that some of the blood on her matches Juno's blood type, prompting Sheriff Vaines and Deputy Rios to take charge and bring along three pothole cave specialists to continue the investigation.

Team heads to the cave via old mine shaft

Vaines, Rios, Sarah, and the three cave specialists Dan, Greg, and Cath are escorted into the cave via Ed's old mine shaft. They intend to locate the missing women and uncover what happened down below. Tension rises as they journey deeper into the claustrophobic tunnels.

Rebecca's corpse found; Sarah experiences flashbacks

Early in the exploration, the group discovers Rebecca's corpse near the cave entrance, triggering Sarah's memories of past events. The grisly find deepens the sense of danger and suspicion that Sarah might be involved in the disappearances. The team presses further into the darkness.

Vaines discharges his pistol in fear when a crawler scuttles nearby, causing part of the cavern to collapse and bury Cath beneath rocks. The collapse separates Cath from the others and blocks their path. They commit to finding an alternate route to reach her.

Bones room yields Holly's camera; victims confirmed

In a room lined with bones, they find Holly's damaged video camera among the debris. The recovered footage shows that the missing women were attacked by crawlers, reframing the mission as a rescue operation rather than a mere search. The discovery raises the stakes and explains the grisly surroundings.

Memory recovery and Dan's death

Hearing the playback, Sarah regains her memories and realizes the truth of what happened. She quietly moves to protect Rios, covering her mouth to prevent a crawler from detecting them. A crawler kills Dan and drags his body away, underscoring the danger in the cave.

Cath escapes; Greg sacrifices himself; Cath dies

Cath squeezes through and kills a crawler by crushing it with rocks. She and Greg maneuver through the treacherous path and encounter Samantha's body hanging across a chasm. They attempt to swing across using Samantha's body, but a crawler attacks; Greg sacrifices himself to delay the creatures, and Cath is killed when she screams Greg's name.

Juno resurfaces; Vaines saved; alliances shift

Vaines is attacked by a crawler but saved by Juno, who is alive. Sarah and Rios reunite with them, shocked to find Juno still among the living. Juno is furious that Sarah left her to die and confronts the group about past actions.

Feeding pit and the severed hand plan

Juno leads the survivors to a feeding pit she claims contains a passage to the surface used by the crawlers. Vaines is handcuffed to Sarah to prevent abandonment. When Vaines slips, Juno instructs Rios to sever his hand to save Sarah, and the trio plummets as the crawlers close in.

Final showdown at the exit; leader defeated

They reach the exit but are blocked by a horde of crawlers led by a large leader. Greg dies clutching Juno to delay their pursuit; Juno is mortally wounded. Sarah kills the crawler leader, but more crawlers arrive; she creates a distraction by screaming to lure them away and give Rios a chance to escape.
