Fifteen-year-old Genevieve Gage [Linda Cardellini] and her best friend Tiana Moore [Amal Rhoe] live in Helverton, Colorado, where they spend their free time chatting with strangers online. Their curiosity about the world beyond their small town leads them to encounter a user who goes by the screen name “Captain Howdy” [Dee Snider], a figure who seems harmless at first but quickly proves dangerous. The two girls quietly drift toward a party hosted by this mysterious persona, hoping to learn more about the people behind the screen. What begins as a harmless curiosity soon spirals into a nightmare when neither Genevieve nor Tiana makes it home the next morning.

Back at home, Genevieve’s mother Toni Gage [Elizabeth Peña] grows anxious and alerts her husband, Mike Gage [Kevin Gage], a local police officer. His investigation is aided by a younger officer named Steve Christian [Brett Harrelson], who helps search the area and follow leads. As the search intensifies, a grim discovery jolts the town: Tiana’s car is pulled from a lake, and inside lie evidence of a brutal killing. The case takes a sharper edge when Mike learns that Genevieve had a habit of meeting strangers from the Internet, a detail that pushes him to dig deeper into Captain Howdy’s draws and motives.

The story digs into Captain Howdy’s dark world, revealing an obsession with body art—tattooing, piercing, branding, and scarification—that marks his victims and their fate. A crucial breakthrough comes when Mike’s niece, Angela Stravelli [Amy Smart], coins the clue that ties Genevieve’s disappearances to online connections. In a tense online encounter, Mike tries to coax Captain Howdy into inviting him to a party, hoping to catch him in the act. Although that particular plan derails, Mike eventually locates Captain Howdy’s lair and uncovers a gruesome torture chamber. There, he finds Genevieve, naked and bound, her mouth stitched shut, alongside several other teenagers who have suffered the same horrific fate. After a brief confrontation in which Captain Howdy is shot, Mike arrests him and learns the true identity behind the mask: Carlton Hendricks.

Mike believes the case is closed, but a legal twist changes everything. A year later, Hendricks is declared not guilty by reason of insanity and is placed at the Meistrich Psychiatric Institute, only to be released three years after that. He moves back to his old neighborhood, where he presents himself as a timid, apologetic man who relies on his medication to keep the memories of his crimes at bay. Yet the community remains unsettled, and a potent mix of anger and fear brews among residents who cannot forget what happened. An activist group led by Jackson Roth [Robert Englund] and Catherine “Sunny” Macintosh [Barbara Champion] intensifies opposition to Hendricks’s release, arguing that he remains a threat.

Tensions erupt when Roth’s teenage daughter, Kelly [Xanthe Cook], goes missing, prompting Roth to suspect Hendricks is responsible. He and a coalition of neighbors kidnap Hendricks to extract the truth, a tense confrontation that ends abruptly when Hendricks drops his medicine bottle and a car runs over it. The group brutally roughs Hendricks and hangs him from a tree, but a sudden rainstorm weakens the makeshift noose and snaps the rope, saving Hendricks from what seemed a fatal end. The brush with death triggers a transformation—Hendricks reverts to his alter ego, Captain Howdy, this time fuelled by a desire for revenge.

Recovered from the near-death experience, Hendricks unleashes a horrific campaign. He kills Jackson Roth’s wife, Madeline Roth [Leslie Wing], in front of Roth and then abducts him, after which he takes Catherine and other captives. Hendricks continues to torment the victims while Mike and Toni watch the ongoing abuse unfold on Toni’s computer screen, when Hendricks briefly showcases Genevieve in a fresh, brutal scene of torture. The chilling display marks a point of no return in the chase, as investigators who respond to the scene find Genevieve, Roth, Catherine, and several others alive but grievously harmed.

The pursuit leads Mike back to the underground club Xibalba, where a climactic struggle unfolds. Captain Howdy confronts Mike with a large meat hook suspended from the ceiling, and a brutal duel ensues. Mike uses the hook to pin Hendricks to a wall and then punishes him with a burning trap, pouring flammable liquid on the Captain and leaving his ultimate fate ambiguous in a blaze of vengeance.

In the aftermath, the town reckons with the harboring of fear and the long shadows cast by the crimes. The harrowing ordeal of a group of teenagers and their families exposes the vulnerability of a community grappling with online dangers, the fragility of legal outcomes, and the extreme lengths to which fear can drive neighbors to seek justice. The line between predator and protector blurs as Mike, Toni, and those who stood against Hendricks confront a case that tested their resolve and left a lasting imprint on Helverton’s memory.
