Former convicts Imani and Carlos are employed by Albert Schillinger, another ex-prisoner who operates a moving company. They are tasked with clearing out the home of an elderly man named Vern Mueller, who offers a bonus if the job is completed in one night, provided they avoid the basement and attic. While working, Carlos learns that Schillinger was previously associated with the Aryan Brotherhood during his time in prison, though he claims to have left that life behind.
The situation turns dire when they discover that Vern is obsessed with the horrific legacy of Josef Mengele. Vern traps the group inside the mansion, unleashing a feral mutant and kidnapping Imani. In his basement lab, he removes one of her eyes to create a specialized rejuvenation serum.
While searching for her, Schillinger and Carlos encounter Sarah and her mute sister, Anne, who have been surgically sewn together. Despite their efforts to rescue Imani, the mutant attacks, and Vern shoots Schillinger. Carlos attempts to hide in a bathroom, only to discover it is a gas chamber designed for killing failed experiments with Zyklon B. Although Carlos manages to escape and kill the mutant, he collapses and appears to be dead.
In the basement, Vern reveals his true identity as the actual Josef Mengele, who has survived for decades using the serum derived from the optic nerves of suffering victims. He reveals that Schillinger was a high-ranking member of the Nazi ideology long before entering prison, a fact that horrifies Imani. Schillinger pretends to still hold these sympathies to trick Mengele into releasing him, allowing him to save Imani.
As the women and the girls escape, Schillinger stays behind to confront the monster. In a final act of defiance, he carves the eagle tattoo from his own chest before crushing Mengele’s skull. The story concludes with Schillinger fighting off a wave of mutants, while mid- and after-credits scenes reveal that Carlos survived and the trio eventually reunited, having escaped the horrors of the house together.
