George D’Peter [Siddique] alias Adattu George, a powerful gold smuggler from Kerala’s famed Adattu family, is murdered along with his driver in a mystery that rattles the underworld and pulls two determined investigators into the case: ACP Muhammad Easa and CI Isaac John. The crime scene shows a chilling signature—a surgical blade—leading the pair to suspect a doctor and to probe medical stores and hospitals for clues. As the case unfolds, Marco D’Peter, George’s younger brother, visits George’s partners William Davis and Abraham Davis to gauge whether they were involved. When Abraham speaks in a crude, demeaning way, Marco acts on impulse and shoots him dead. At Abraham’s funeral, Easa presses William and his younger son about Abraham’s murder, but the truth remains elusive.

In the shadows of Star Hospital, Patrick, the secretary to the Davises, is gunned down in a high-security setup, deepening the belief that a doctor is behind the killings. Easa and Isaac push harder, but Isaac grows wary of Easa, suspecting that the investigation is being compromised. A breakthrough comes when Isaac uncovers Dr. Michael John [Nivin Pauly], a young surgeon at Star Hospital, as the killer and as Easa’s informant. Isaac pushes for transparency, forcing Easa to reveal his knowledge of Michael’s past and the ties between the doctor and the murders.

The film then shifts into a dark, carefully built flashback to Michael’s childhood. Ten-year-old Michael, urged by his martial arts trainer father John [Babu Antony], takes the wheel of a car, a decision that leads to John’s death and tears the family apart. Michael’s mother, Ancy [Shanthi Krishna], blames him for the tragedy, and the two drift apart. Back in the present, the path of violence becomes personal and spirals further. George’s son, Jerard ‘Jerry’ Peter [Amal Shah], defies expectations by challenging Jenny Jennifer “Jenny” John at school, only to be defeated by her in a kung-fu display. The humiliation triggers a brutal chain of events as Gerald’s circle mercilessly harasses his sister, and George manipulates the situation to push her toward danger, threatening the whole family.

Michael’s past bleeds into the present as George’s vendetta escalates. Ancy’s untimely death in a car collision, allegedly engineered by George, fuels Michael’s rage. The pressure and pursuit grow more vicious: George’s forces brutalize Michael, ending with him left on a railroad track, a perilous moment that mirrors his later reckoning. Michael survives the ordeal, and a personal score is settled when he finally catches up with George and his driver, ending their reign with ruthless intent.

As the present-day conflict intensifies, Marco learns of Michael and Jenny and swears to complete what he started. Easa reveals that he ordered Michael to kill Patrick to ignite a gang war, a revelation that rattles the already-volatile underworld. William [Jayaprakash] and Francy Davis [Sudev Nair] kidnap Jenny, hoping to force Michael into the open. But Michael arrives just in time to mount a rescue, and the would-be criminals are detained by Easa, who also arrests Marco for Abraham’s murder. A bail allows Marco to walk free, but his luck runs out when he abducts Jenny and takes her to a helipad atop a skyscraper, challenging Michael to a fatal test of skill.

The final confrontation unfolds on the open-air pad. Michael [Nivin Pauly] confronts Marco Jr. [Unni Mukundan], and a brutal hand-to-hand duel ends with Michael delivering a decisive, injurious strike. In a final, chilling display of control, Michael uses a hidden surgical blade to finish the fight and signs Marco up for organ donation, securing Jenny’s safety as he walks away with her, their future uncertain but free from Marco’s immediate threat.

The film blends a meticulous procedural investigation with a sobering meditation on the cycles of vengeance and the scars left by mistrust.
