Travis Dee fights his way through a brutal onslaught of the undead, but a splash of infected blood hits his face and seeds a terrifying transformation. He clings to the belief that a rogue, experimental bio-toxin is the culprit and tries to convince the press and the police that the outbreak is man-made rather than natural. The authorities, including the Centers for Disease Control and Prevention, are divided and suspicious, and ultimately he is sent away to Haywood Maximum Security Prison to await further testing and evaluation.

A BBC correspondent, Samantha Beckett, is dispatched to gather more information about the escalating crisis and to observe how the prison handles the alarming reports. As Travis and a fresh wave of inmates arrive, they are processed and lined up under the watch of Lieutenant Sweeney as he declares that he is “top of the food chain” in this harsh facility. When he notices Travis’s severe coughing and pale appearance, he orders him to the infirmary, suspecting nothing less than a full-blown outbreak.

The prison also welcomes a new arrival, a man named Johnny, who is brought before Warden Mahler. Mahler issues a stark warning not to provoke trouble or anger the guards. Johnny’s story mirrors Travis’s, claiming that the danger is real and that an infection is spreading. A bitten security guard dies, and the prison’s leadership—Sweeney, Dr. Goring, Samantha Beckett, and Johnny—scrambles to determine what exactly is happening and whether the virus exists outside rumor.

In the infirmary, the medical team led by Dr. Goring struggles to verify the truth of the outbreak. The evidence grows murky when Samantha examines Travis’s body and uncovers a disturbing clue: the blood on his skin suggests he died hours earlier, even though Sweeney himself had shot him only moments before. The tension among the command staff thickens as they debate their next moves and contend with the fear that they might be dealing with something far more dangerous than a conventional infection.

A plan forms among the guards: Sweeney, Jenkins, Mahler, and a team including Keith decide to move through the building, gather all uninfected staff, and barricade themselves in the staffroom at the far end of the compound. Meanwhile, the guards outside will do whatever it takes to kill the infected. The proposal to abandon the uninfected inmates weighs heavily on Johnny, who disagrees with such cold calculations. He makes a desperate choice, disarming a guard and racing toward the outside in an attempt to stop the chaos.

Samantha and Johnny manage to reunite and decide to make an escape together, recognizing that survival depends on one bold move rather than a stubborn stand. They slip away through the chaos, aiming for the exterior and a chance at freedom. But the disease does not care for plans; Johnny is bitten in the midst of the escape, and Samantha, determined to give him a chance, keeps him hidden long enough to buy a critical window of time.

Back in the compound, the situation deteriorates as Sweeney becomes utterly consumed by the infection. He turns hostile and begins shooting at anyone nearby, including a fellow guard. The fight intensifies as the remaining personnel try to reclaim control, but the outbreak has already rewritten the rules of engagement.

In the end, Samantha is forced to flee as the danger closes in. A sniper mistakes her for a zombie and delivers a fatal shot, cutting short her attempt to escape. The yard scene reaches a quiet, grim conclusion when Jenkins is bitten and is put out of his misery by Samantha. The prison’s last stand collapses under the weight of the outbreak, and the surviving officers must confront a reality in which the threat is no longer contained but out in the open, waiting to be faced once more.
