Kaalaiyan is a village sarpanch in Soorakkottai, Thanjavur District, who leads with fairness and a strong sense of justice. He finds himself at odds with a ruthless rival, Nattadurai, who rules through intimidation and fear. When Nattadurai tries to seize land from an elderly man, he hires lawyer Pattammal to defend the case. [Kaalaiyan] exposes the truth, but Pattammal betrays her client and withdraws the case, setting off a dangerous turn of events. Despite the betrayal, Pattammal and [Kaalaiyan] slowly develop feelings for each other, a bond that will redefine their loyalties.

Within his close circle, [Kaalaiyan] is deeply devoted to his sister, Thanga Meenakshi, whom he raised after their parents’ death. When Meenakshi returns home after completing her education, finding a suitable husband for her proves tricky, because many fear the village leader who stands by fairness above all. The two meddling cousins, Angayarkanni and Mangayarkarasi, press their brothers to marry Meenakshi off to calm the family dynamics. Against her will, Meenakshi’s marriage is arranged with Nattadurai’s younger brother. On the wedding day, Meenakshi vanishes; a search party eventually locates her on a train, attempting to elope with her college boyfriend.

Torn between his love for his sister and his principles, [Kaalaiyan] allows Meenakshi to pursue her own path and resists without cleaving to family demands for an honor killing. Meenakshi and her husband relocate to Kolkata, and she gradually loses contact with her brother. Determined to stand by her, [Kaalaiyan] and [Pattammal] travel to the city, where he learns of Meenakshi’s dire financial straits. He discovers she has borrowed money from a gangster and moves to protect her, secretly rescuing her from threats and arranging a safer refuge at [Pattammal]’s apartment. When thugs close in, Meenakshi falls ill, and Pattammal leaves briefly to fetch medicine while the danger escalates; the attack on them intensifies, and [Kaalaiyan] must intervene to uncover who is tormenting Meenakshi.

In Kolkata, a wealthy businessman and underworld don, Manoj Palekar, seeks to buy a controlling stake in Meenakshi’s business. When her husband refuses, a chain of calamities follows: Meenakshi’s factory catches fire, her husband is arrested on fabricated charges, and Manoj’s men murder anyone who helps her. This oppressive shadow from the past resurfaces as Meenakshi fights to keep her family afloat under immense pressure and fear.

Back in the present, Meenakshi discovers she is pregnant, and [Kaalaiyan] rejoices at the news. To celebrate and ease her mind, Pattammal and [Kaalaiyan] organize a baby shower. Fueled by the betrayal and the earlier losses, [Kaalaiyan] vows to take down Manoj and the gangsters who ruined Meenachi’s life. Under the pseudonym “Annaatthe,” he and his village allies storm Manoj’s office and compel him to sign papers, restoring the stolen companies to their rightful owners. Although Manoj is spared, his pride shatters, and he later confronts his estranged brother Udhav Palekar, who now demands the death of Annaatthe and ultimately takes his own life.

The pursuit of Meenachi intensifies as [Udhav Palekar] leads fighters to attack, forcing Pattammal to hide Meenatchi. In the ensuing chase, Pattammal and Meenatchi are separated by a crashing auto-rickshaw, and [Kaalaiyan] arrives just in time to battle the attackers. He defeats the assailants, and the film closes on a poignant note as [Meenatchi]—who has endured so much—opens up to [Kaalaiyan], tearfully apologizing for the trouble she caused. Through courage, loyalty, and a relentless fight for family, the story honors a man who chooses fairness over fear and a woman who survives against the odds, finding strength in the man who stood by her side.
