Denis Dimbleby Bagley, Richard E. Grant, is a mentally unstable advertising executive whose nervous breakdown erupts during a shoot for a pimple-cream commercial. His wife Julia Bagley, Rachel Ward, is a patient and supportive presence, while his boss John Bristol, Richard Wilson, watches the chaos with a mix of exasperation and curiosity.
A crisis of conscience about the ethics of advertising triggers Bagley’s slide into mania. Out of this turmoil, a literal growth on his right shoulder—a boil—forms a life of its own. The boil is voiced by Bruce Robinson and quickly adopts a cynical view of the industry that created Bagley’s problems.
Bagley, seeking relief, moves toward surgery to remove the boil in a hospital, but just before the operation, the boil-head grows into a full replica of Bagley’s head — complete with a moustache — and, in a swift reversal, covers Bagley’s own head. The doctors lance the boil head, unaware of the swap, while the original head is left effectively as a boil on his left shoulder.
Now, with the boil-headed version in control, Bagley returns home to celebrate a wedding anniversary. The original head remains a living boil on his shoulder, while the new, moustached head embodies Bagley’s renewed, ruthless persona. The boil head withers but does not die, and Bagley climbs back into advertising life, sharper and more calculating than before, even as his wife leaves him and their relationship is irreparably strained.
Across the film, the clash between Bagley’s ethical awakening and the boil’s biting cynicism fuels the farce, blending satirical wit with physical comedy. The story tracks how a crusade for integrity can be co-opted by ambition, leaving the central figure to navigate the cost of success while his marriage dissolves.
