Eight years have elapsed since the events of Ek Tha Tiger, and the fates of Tiger (Salman Khan) and Zoya (Katrina Kaif) remain a mystery. In Ikrit, Syria, a group of 40 nurses—25 Indians and 15 Pakistanis—are being held hostage inside their own hospital by Abu Usman, the ruthless leader of the terrorist organization ISC. The Research and Analysis Wing (RAW) chairman, Shenoy (portrayed by Girish Karnad), believes that only Tiger can bring back the nurses, particularly as the United States has planned an airstrike to eliminate Usman within a tight timeframe of 7 days following the execution of a CIA operative.
Currently, Tiger and Zoya reside in Innsbruck, Austria, where they have a son named Junior. After discovering Tiger’s location, Shenoy and his deputy, Karan (whose role is pivotal), approach him with this desperate mission. Initially reluctant, Tiger ultimately agrees, encouraged by Zoya’s urging to serve their country. Along with RAW agents Azaan (Paresh Pahuja), Namit (Angad Bedi), and Rakesh (Kumud Mishra), Tiger embarks on a harrowing journey to Syria. Shenoy, concerned that Tiger may have strayed during his years abroad, instructs Karan to closely monitor him.
To gain access to the hospital, Tiger’s team devises a risky plan to infiltrate an ISC-controlled refinery disguised as illegal immigrants. They aim to create a diversion through a controlled explosion, gaining entry to the facility under the pretense of being injured. At the refinery, they encounter Firdauz, or Tohbaan (Paresh Rawal), who is a key supplier of illegal labor and harbors distrust toward the newcomers. Meanwhile, through his resourcefulness, Tiger manages to save a young boy caught in the grips of ISIS extremism, showing his innate compassion even amid chaos.
As the plot thickens, Zoya learns about the Pakistani nurses, prompting her to join forces with Tiger, alongside ISI operatives Abrar (Gavie Chahal) and Javed.
