Betty Lou Perkins, Penelope Ann Miller, is a meek librarian in New Orleans who is ignored by everyone, including her uninspiring police detective husband, Alex, Eric Thal. A criminal kingpin is killed in cold blood, and Betty Lou finds the murder gun, a discovery that sets a chain of risky events in motion. She goes to the police station to tell them about the gun, hoping for a sympathetic ear, but no one listens until she actually fires the weapon at a trash can in the bathroom. However, when the other characters—including Alex—don’t know she can hear them, they all conclude she is innocent because she’s too boring and timid to ever do anything at all, let alone anything wrong. A fed-up Betty Lou finally declares she killed the guy and is arrested, to the complete shock of everyone who discounted her.

Behind bars, Betty Lou meets a variety of hardened and colorful characters. Rather than intimidate her, they actually raise her self-confidence and alter how she sees herself. Once she is released, she begins to dress, speak, and act differently, embracing a newfound boldness that others find unsettling. Unfortunately for her, mob-connected acquaintances of the victim assume she must have confessed for a reason, and they quickly conclude she must be his mistress, bringing a dangerous aura of suspicion and menace into her life. They want a few words with her—or worse. Meanwhile, Alex very slowly realizes he has been a worthless husband, and there’s a real threat to his wife unless he can stop the bad guys, setting up a tense dynamic as he confronts his own failures while trying to protect the woman he once overlooked.
