Sam Walker, a NASA astronaut from the International Space Station, crash-lands back on Earth after a mission goes wrong when her shuttle is struck by debris. Rescued by the U.S. Coast Guard, she is taken to hospital for recovery. Upon awakening, doctors in hazmat suits enter the room to question what happened during the accident; Sam does not remember and is placed into quarantine. She is visited by her husband Mark Walker and daughter Izzy before her father, Gen. William Harris, arrives to tell her the Pentagon is now taking over the investigation. For her safety, Sam is confined alone to a rural cottage.
As Sam adjusts to her new situation, she undergoes a series of medical tests to ascertain her mental and physical fitness. She calls Mark, who explains the family is still struggling to cope. While walking outside, Sam notices the premises are wired with hidden cameras, and then she trips and passes out. When William arrives with the scientists, they conduct more tests. Mark and Izzy also visit. Sam privately tells Mark about the black creatures and reveals the true purpose of her mission: to oversee the transmission of a message meant for extraterrestrial life. She believes something detected the message and followed her back to Earth. A swarm of insects later attacks the family, covering the building before dying en masse.
Sam begins to show bruises and a worsening condition. Again, at night, the power cuts out and the alarm signals a breach. Sam retreats to the bunker, only to discover her body is covered in a placenta-like material. Three creatures already inhabit the house, and when they enter the bunker, Sam flees through the air ducts into the woods, where the creatures confront her. Through telepathy, they reveal they are her real family and that Sam herself is an alien who had lived as a human girl after her ship crashed years earlier; the disguise is wearing off. Mark and Izzy arrive after a phone call. William confronts them, revealing that Sam’s mission was designed to lure the other aliens back so they can be captured by the government. Before he can take Sam into custody, Mark knocks him out.
Sam’s body fully deteriorates, revealing her true alien form to Mark and Izzy. Accepting that her human life is over, Sam says goodbye to her Earth family and boards a spaceship with her real family to escape the converging authorities. As the ship rises, William, Mark, and Izzy watch it disappear into the night sky.
