[REC] 4: Apocalypse 2015

A Special Forces Team is sent to an apartment complex to rescue a reporter named Ángela and destroy the building, navigating a tense aftermath that turns the mission into a fight for survival. After an overwhelming assault from infected firefighters and residents, the only obvious survivors are Lucas and Guzman, who manage to complete the assignment even as the place falls into chaos around them. The narrative then shifts to a larger, more ominous stage: Ángela awakens on a ship, chained to a bed, with her memory drifting and no clear explanation for her predicament. A medical team led by Dr. Ginard takes a careful blood sample from her, while Lucas and Guzman remain aboard the quarantined vessel, cut off from the outside world.

As the ship holds its claustrophobic lockdown, a failed attempt to escape leads to a crucial revelation: Ángela is told by Dr. Ricarte that she is not infected. Yet the ship’s atmosphere grows increasingly claustrophobic as Guzman forges uneasy connections with the captain Ortega and a technician named Nic, whose role becomes pivotal in piecing together what happened in the apartment via the footage captured by Ángela’s camera. Nic transmits the reconstructed film to the doctors in the laboratory, and their grim investigation into the pathogen begins in earnest, revealing a chain of contagion that stretches far beyond a single building.

The doctors discover a chain of accident and experimentation: their test subject, an infected monkey, had been released during a power outage, contaminating the ship’s cook, who then taints the food supply. Ingestion spreads the infection to much of the crew and the guards, eroding any chance for a quick cure. When an antidote proves unable to cure the cook, the group—Ángela, Guzman, Lucas, and an elderly woman who survived an outbreak at a wedding—head toward the bridge to seek safety or a solution. Along the way, the elderly woman is separated from the group, and Lucas makes a harrowing decision to stay behind and rescue her, only to discover she has become infected and must be shot to prevent further spread.

Ricarte soon realizes that the attempt to find an antidote has failed because it requires an unmutated strain of the pathogen, a detail that complicates every rescue attempt. Just before he can trigger a self-destruct sequence, his assistant reveals the reconstructed film, which shows Ángela being orally infected with a worm-like parasite by the pathogen’s original carrier. This revelation spurs a desperate struggle: Ricarte attempts to surgically extract the parasite, but Ángela is saved when an attack by the now-infected Ginard disrupts the procedure and allows her to escape. In a tense confrontation, Ricarte pursues Ángela, who ambushes him and delivers a fatal bite.

A negative blood scan does little to reassure Ricarte, who remains convinced that Ángela is deceiving him. In a cruel twist of fate, the parasite transfers to Guzman during the rescue, trapping Ángela in the bowels of the ship as Ricarte initiates the self-destruct sequence. Lucas and Nic struggle to power the ship’s auxiliary engine, but the effort fails, and Lucas is killed by an infected survivor. Nic then discovers Ricarte attempting to flee in a lifeboat, incapacitates him, and seizes the lifeboat for his own escape in search of Ángela.

Nic eventually finds Ángela on the run from infected monkeys, and they fend off the creatures with an outboard motor. The tension escalates when Guzman reappears, intent on transferring the parasite back to Ángela. In a final, decisive encounter, Nic and Ángela kill Guzman with a harpoon gun, and the two of them flee the collapsing vessel as the infected outer corridors close in. They jump into the ocean just as the ship explodes behind them, and the parasite—still a threat—twists underwater before being swallowed by a fish. In the aftermath, the credits show Ángela and Nic back on land, sharing a moment of fragile relief as they head toward home.
