A Caribbean coastal resort, Hotel Elysium, finds itself menaced not by storms but by a deadly creature feature: flying piranhas spawned from a sunken shipwreck that lurks just off the coast.
Anne Kimbrough [Tricia O’Neil] is a diving instructor at the hotel who notices something off when a student becomes the latest victim. Her estranged police officer husband, Steve Kimbrough [Lance Henriksen], refuses to let her see the corpse, even as she knows the death wasn’t caused by a shark or barracuda—her marine biology training tells a different story, one that unsettles the couple’s fragile past. Soon after, a string of killings—two women and a man—confirms her fears: the attackers are piranhas that have somehow evolved the ability to fly.
Concerned and stubborn, Anne fixes her sights on a tourist, Tyler Sherman [Steve Marachuk], who constantly bugged her and who she tries to bring along to the morgue to inspect the body. She explains her expertise, which gives her credibility about what didn’t kill the victim, but the encounter ends with a nurse ejecting them and, ominously, a piranha hiding in the body and slipping away. In the wake of the chaos, Anne and Tyler share a one-night stand. The next day, Anne studies the photographs of the corpse and is horrified by what she finds. Steve arrives, throwing the credit card she left behind in a fit of anger, furious that she went to the morgue and that there was another man in her bed. She attempts to warn him, but he remains unconvinced.
When Anne tries to cancel diving sessions, her manager fires her in a blow to her livelihood. In a bid to prove the threat, she aims to capture a specimen, only to be intercepted by Tyler, who reveals he is a biochemist and part of a team that has created a weaponized, genetically modified piranha capable of flight. He explains that a cylinder of these fish was lost earlier, still somewhere in the water.
Gabby [Ancile Gloudon] provides crucial proof by showing that the piranhas are now eating each other, underscoring the peril. Anne attempts to share what she knows at a managers’ meeting, but the room is skeptical, and Steve dismisses her warnings as the ravings of a frightened lover. The danger escalates when a piranha attacks Gabby’s son, killing him and driving Gabby to vow revenge. The hotel’s nighttime “fish party” to catch grunion, hosted by the manager Raoul [Ted Richert], unintentionally becomes a feeding frenzy as the piranhas crash the party. Anne enlists Aaron [Aston S. Young] to patrol the beach, but he is lured toward the sea and killed.
As the piranhas swarm the guests, Anne and Tyler race to the wreck, where the predators push the crisis to a fever pitch. They lead survivors into the hotel, trying to seal themselves behind doors and windows, while Gabby fights the voracious intruders and is overwhelmed. By morning, the piranhas retreat with the daylight, leaving Anne and Tyler to a drastic plan:Gabby’s final scheme to blow up the sunken ship and wipe out the flying fish.
Meanwhile, Steve’s son Chris Kimbrough [Ricky Paull Goldin] has been hired, despite his parents’ wishes, by a local ship captain, and Allison Dumont [Leslie Graves], the captain’s daughter, join him on a voyage that ends with them adrift on a raft above the wreck. Anne and Tyler race to the wreck in a motorboat, planting Gabby’s timer charges. With only ten minutes to escape, they find themselves trapped in one of the ship’s rooms as the piranhas close in. Steve, piloting a police helicopter, ditches the chopper and swims to the motorboat, then powers it away toward safety with Chris and Allison aboard. Tyler becomes trapped in the wreck and is consumed by the hungry fish. Anne escapes through a porthole and grabs the ship’s anchor to haul herself free as the motorboat pulls away.
The explosion racks the sunken ship, annihilating the piranhas and sealing the fate of the predators. Surfacing, Anne is rescued by Steve, Chris, and Allison in their boat, the immediate danger over for now, the island’s coast quiet once more, and the couple’s bond tempered by catastrophe but kept intact by survival.
