Mathew Harris, Paul McGann, is a porter in a busy London hospital who secretly dreams of bigger opportunities. He spends his evenings strumming a banjo in his cramped bedroom, still smarting from a choice he once made to drop out of biology studies. One fateful night, he witnesses a car crash outside a local pub—the driver is killed in a collision with a lorry. Harris knows the dead man, Simon Hennessey, a doctor at his hospital, and soon realizes Hennessey had been pursuing a residency at another hospital in Bristol. Driven by ambition and a hunger for a fresh start, Harris seizes the moment: he takes the deceased man’s mail, crafts a job application, and applies for the position under the late doctor’s identity. After poring over medical texts and quietly observing procedures at his own hospital, Harris is accepted for the new role.

Before departing, he seeks out his friend Alec Moran, Jimmy Yuill, at the pool and explains he’s traveling abroad. To keep the illusion, he hands Moran his silver cigarette case and quietly quits his job, heading to Bristol where he will take on the alias Dr. Simon Hennessey, Dale Rapley, in a bustling Accident & Emergency (A&E) department.

In Bristol, Harris’s inexperience is tempered by a mounting confidence that comes with authority. He faces his first real tests as a physician and is quickly reprimanded by a supervisor for sloppy patient record-keeping, a fault that threatens to derail his bold new life. He agrees with Taylor that the impersonation must end and that they should move toward a life in London together. Yet in a final, stubborn move, Harris heads toward Salisbury instead of following through with the plan to vanish into a new life with Taylor.
