Omkara “Omi” Shukla Ajay Devgn is an enforcer for a gang that carries out political crimes on behalf of the local politician Tiwari Bhaisaab Naseeruddin Shah. His two closest lieutenants are Ishwar “Langda” Tyagi Saif Ali Khan and Keshav “Kesu Firangi” Upadhyay Deepak Dobriyal. The trio keeps a tight, violent order, enforcing the will of their ruthless boss while navigating the murky lines between loyalty, ambition, and survival.
The story kicks off with a dramatic clash at a wedding—Langda gatecrashes a baraat and openly challenges Rajan “Rajju” Tiwari Deepak Dobriyal to try stopping Omkara from abducting the bride, Dolly Mishra Kareena Kapoor Khan. Rajju fails, and the wedding never takes place. Dolly’s father, advocate Raghunath Mishra Kamal Tiwari, is furious and feels a deep sense of betrayal. He threatens Omkara, and the fallout forces Bhaisaab to intervene. Dolly clarifies that she eloped with Omkara rather than being abducted, but the stain on Omkara’s name remains. Raghunath Mishra leaves the village in shame, and the family’s reputation is bruised.
As the political game heats up, Omkara and his gang strike a calculated deal with Indore Singh, a rival to Bhaisaab, by exploiting a sensational MMS sex scandal. They eliminate several of Singh’s enforcers while letting one, Kichlu Pankaj Tripathi, live to carry out further jobs. The political landscape shifts: Bhaisaab is elected to Parliament, and Omkara is promoted as a candidate for the upcoming state elections. The power grab deepens the web of influence and danger surrounding them.
Omkara decides to crown Kesu as his successor in politics, appointing him over Langda because Kesu, with his upper-class background and college education, is seen as better suited to win over younger, educated voters. Langda’s jealousy and sense of being undervalued grow into a consuming vendetta. He engineers a rift by stoking a quarrel between Kesu and Rajju, exploiting Kesu’s low tolerance for alcohol to undermine Omkara’s trust. Langda further manipulates Kesu into visiting Dolly and uses those visits to feed Omkara a story of an alleged affair. He also orchestrates Indu, his wife and Omkara’s sister Konkona Sen Sharma, to steal a precious piece of jewelry that Omkara had given to Dolly, and then nudges Kesu to gift it to his girlfriend Billo Chamanbahar Bipasha Basu.
Billo’s presence is leveraged to draw Kichlu out of hiding, enabling Omkara’s gang to assassinate him. The violence intensifies as Omkara, Langda, and their associates carry out brutal killings on a train. In a moment of raw tension, Omkara assaults Langda and presses him to reveal whether Kesu and Dolly are truly involved. Langda coldly confirms their affair, feeding Omkara’s deepest suspicions.
On the couple’s wedding day, a bird drops a snake into the paint being used on Dolly—an ominous omen that foreshadows the tragedy to come. Indu warns her brother not to marry Dolly if he harbors doubts, a warning that only heightens Omkara’s uncertainty. Langda continues his manipulation, tricking Omkara into believing that Kesu’s conversations about Billo refer to Dolly, and stages a scene in which Omkara watches Billo drop the heirloom jewelry at Kesu’s door. Convinced of an affair, Omkara smothers Dolly to death on their wedding night. In the same breath, Kesu is shot by Rajju, who collapses but survives, and Rajju himself shoots himself. Hearing the gunfire, Indu rushes to Omkara and discovers Dolly’s corpse along with the recovered jewelry. She confesses to having stolen the jewelry for Langda, exposing the web of lies that has ensnared Omkara.
