A young girl named Akiko Michiyo Yamazoe loses her dog while strolling along a lonely beach, and the chase pulls her into a towering European mansion. Inside, an old man watches her with a fixed gaze as she pursues the dog deeper into the gloomy halls. There, she stumbles upon a room where a dead woman lies by a piano, and it is in that moment that she first encounters the mysterious vampire Shin Kishida. The encounter is brief but searing, leaving a mark on Akiko that will echo through the years.
Eighteen years pass, and Akiko reappears as an adult, now played by Midori Fujita. She lives in a quiet lakeside setting, still haunted by what happened at the mansion and increasingly convinced that what she experienced was not merely a dream. Her life begins to braid with the strange and unsettling when Kusaku, a local boat operator, receives an enigmatic package—a white coffin. He voices his concerns to the shipping agent and, upon returning to investigate, discovers the coffin is empty and is abruptly attacked by the same vampire Akiko once saw years ago.
Meanwhile, Akiko’s partner, the doctor Chōei Takahashi who plays Dr. Takashi Saeki, is drawn into the strange events when a woman is admitted to the hospital with two bite marks on her neck and then dies. This escalating mystery tugs at Akiko’s life as her sister Natsuko Kashiwagi Sanae Emi disappears into the same eerie atmosphere. Akiko searches frantically and finds her dog’s body in a field, near which Kusaku loiters. He lunges at Akiko, sparking a desperate chase that ends with her being knocked out by a falling branch and carried away to his house. Just as a vampire closes in for a bite, two fishermen at the door disrupt the moment, forcing the intruders to retreat into the shadows.
Back at the hospital, the tension intensifies: Takashi confronts a patient who has been drawn toward the vampire’s influence, and the hospital becomes a stage for fear as the vampire manipulates the environment. Takashi notices the patient’s strange behavior, yet tragedy strikes again when she falls to her death after an unsettling confrontation. The threat reaches Natsuko as she returns to Akiko’s home, bringing the vampire with her. Akiko hides in a closet as Takashi is ambushed in his car by Kusaku; a crash ensues, and Takashi survives while Kusaku is killed. Returning home, Takashi and Akiko learn that Natsuko has died on a beach, and, with her dying breath, she begs to have her corpse burned. They take her to the hospital for an autopsy.
In the morgue, Natsuko unexpectedly rises from the dead and escapes, while Takashi uses hypnosis to push Akiko into reliving her “dream.” This moment reveals that the childhood incident really happened, and it helps them understand why Akiko’s life has felt so haunted. Together, they decide to return to the mansion from the past to uncover the truth. There, they discover the dead body of the old man who once lived there and a diary that unpacks a long, chilling history. Although the old man himself was not a vampire, his father built the house in a remote part of Japan to shield his family from a vampire lineage. The old man’s son, not the father, becomes the vampire, and a tragic sequence unfolds: a young Akiko and her dog encounter him, they escape, and the father locks the son away. The son eventually breaks free and feeds on his own father, who leaves behind a diary before dying of the wounds. This diary is a key to the unsettling truth.
Armed with the diary’s revelations, Akiko and Takashi confront the current vampire and Natsuko in a climactic battle on a balcony. The old man, now reanimated as a vampire himself, seizes the vampire’s leg, causing him to crash through weakened wood and collapse, turning to dust as the curse breaks. Natsuko, too, collapses at the moment the curse lifts, and both she and the old man return to ordinary corpses. With the curse lifted and the past laid to rest, the haunting presence that once tethered Akiko’s life finally dissolves, leaving her and the people around her to move forward with a clearer sense of who they are and what they can endure.
