Five American backpackers — Gina, Emma Catherwood; John, Cian Barry; Stacey, Lisa Livingstone; Geraldine, Jane Perry; and Phil, Michael Smiley — hike through dense woods in India with their guide Brian, Mike Rogers. When a venomous spider bite strikes Geraldine, the group decides to seek help from an American doctor living with a jungle tribe, Dr. Lecorpus, Lance Henriksen. Dr. Lecorpus treats Geraldine with the aid of his brother, who wears a cloth over his disfigured face, and the two become central to a broader, darker plan.
As Gina, John, and Phil return toward civilization, Brian and Stacey stay behind with the natives and press on to a temple deep in the forest while Geraldine slowly recovers. The separation forces the others to navigate a maze of forest paths and ceremonies, and tragedy soon follows when Stacey is ensnared by a vast swarm of spiders that envelop her in webs.
Gina, John, and Phil head to a nearby village and then to the police station, where Sgt. Chadhri begins a formal inquiry. Chadhri heads into the woods and spots Lecorpus and several natives near a helicopter, but is suddenly seized by Lecorpus’ brother; the gun is taken, and Chadhri is forced to walk back to the village. Undeterred, Chadhri agrees to investigate the temple, and is lured deeper into danger when Lecorpus’ brother launches a brutal assault that sends him fleeing into the temple’s shadowy corridors.
Meanwhile, Gina, John, and Phil grow anxious about Chadhri’s disappearance. With Gina and John returning to the woods, they witness a chilling ceremony at the village. Stacey, still encased in webs, is injected with venom and carried off into the temple by Lecorpus’ brother. The natives mount a counterattack to rescue Stacey, and Gina discovers Stacey still alive but paralyzed. Lecorpus appears and claims Stacey is not dead, simply incapacitated, and Gina and John race into the temple, only to be separated.
Inside the caves, Phil uncovers a troubling lead—a newspaper report implying Lecorpus’s illegal organ harvesting. Gina and John find Stacey’s camcorder and discover an alive but dying Brian, who passes away soon after. The trio reunites with Chadhri, and they push on to locate an operating room where Lecorpus, his brother, and several natives are conducting a chilling scheme to harvest organs from travelers. Victims are venom-injected to prevent spoilage during transport, a cruel and methodical process hidden beneath the forest’s shadows.
Gina, John, and Chadhri manage to free Stacey and escape the operating room. They sprint through a labyrinth of tunnels while being hunted by Lecorpus’ brother and the natives. A fire erupts in one passage, killing Lecorpus’ brother, and the refugees scramble across a spider-web bridge to flee a looming chasm. Chadhri leads, followed by Stacey and then Gina. John attempts to mount the web, but it cannot bear all their weight; he is left stranded as Gina and Lecorpus clash above the chasm, and a plunge seems imminent.
Gina swings across and hurls Lecorpus into the chasm, where spiders swarm and kill him. Gina, Stacey, and Chadhri escape the temple, while Lecorpus’ brother attacks in pursuit. Phil and a squad of police arrive, shooting the attacker dead and reuniting with Gina and Stacey. They reveal that Brian and Geraldine have died, and they warn that John remains trapped inside, still entombed in spider webs as the danger outside slowly subsides.
Sometime later, Gina returns to the village and is visited by Chadhri, who updates her on Stacey’s improving condition and the ongoing search for John. Yet the film closes on a bleak note: John is shown completely encased in spider webs, a haunting image that hints at a fate sealed within the temple’s labyrinthine depths.
