Suddenly, tragedy strikes: Tim and Sandra go inside to retrieve wine, leaving Ginny and Phil alone outside. In a cruel turn of events, the Butcher appears and viciously attacks Tim, shoving a wine bottle down his throat and smashing it for devastating effect, causing fatal injuries. He then murders Sandra by smashing her head with a toilet seat, and soon after, he finds Phil, impaling his head with a broken tennis racket, before finally restraining Ginny with a rod in her own living room. The teenagers’ horrifying experience is interrupted only when Ginny’s parents come home to discover her shattered body, with the mystical dagger mysteriously missing from the scene.

The story then shifts to Thursday the 12th, focusing on Millie Kessler, played by Kathryn Newton, who is waking up to another typical day at school. Millie lives with her mother Paula and her older sister Charlene, a cop. It’s been a year since their father passed away, and the emotional strain on the family is palpable, affecting their relationships. Millie, struggling with self-esteem, tries to navigate her daily life while facing teasing and hostility from classmates. Later that evening, during a school football game, Millie is acting as the team mascot when she is unexpectedly cornered after soda is thrown at her. As she waits for her mom to pick her up, the lights around her flicker and go out, and she is suddenly pursued by the Butcher himself. The killer, wielding La Dola, raises it high before stabbing Millie in the shoulder. Her sister Charlene arrives just in time to shoot at the Butcher, who flees into the night, leaving Millie injured and visibly different afterwards.

On Friday the 13th, Millie wakes up to find herself in the body of the Butcher—a terrifying and shocking transformation. Meanwhile, the Butcher wakes up in Millie’s body, confused and aggressive, wandering around. Millie, trapped in the killer’s form, panics and tries to seek help but quickly realizes that everyone recognizes the Butcher’s face from a recent sketch on the news. Her first foray into her new body is chaotic—she attempts to hide and adapt, but encounters violent confrontations. She is mistaken for the killer and ends up in a freezer, where her friend Nyla’s body (whose murder she witnesses) shatters, adding to the chaos.

She then manages to convince Josh and Nyla of her true identity by performing a cheerleading routine only Millie would know. Their urgency grows as they learn from a Spanish teacher about La Dola’s curse: the transformation becomes permanent after 24 hours. Time is slipping away, and Millie’s friends rally to locate her original body before it’s too late. They realize that the switch is only reversible with the dagger, which they must retrieve from the police evidence locker.

Throughout their frantic efforts, Millie, in the Butcher’s body, navigates a perilous path—threatening her friends with intimidation, fighting off attackers, and trying to prevent the Butcher from murdering more people at the upcoming homecoming dance. Meanwhile, her friends try to keep her hidden, disguising her with a mask and attempting to get her back into her own body.

As the climax unfolds at the old mill during the dance, the Butcher, in Millie’s form, commits brutal acts—including attacking other students and wielding a chainsaw. Meanwhile, her friends manage to tie him up temporarily in an effort to set things right. They realize they are running out of time, as the alarm set on Booker’s watch indicates mere minutes remain before the transformation becomes permanent. In a tense race against the clock, they finally succeed in forcing Millie back into her original body by stabbing La Dola into the Butcher’s form, ending his reign of terror just in time.

Even after the killer’s death, the story doesn’t fully conclude—the Butcher surprisingly survives and begins attacking Millie’s family at home. Despite her efforts to fight back, he incapacitates her mother Paula and Charlene. In a desperate final act of bravery, Millie confronts her adversary by exploiting his vulnerability. She kicks him—delivering a decisive blow—and uses a broken table leg to impale him, ensuring his demise. Standing victorious, Millie declares her resilience, saying, “I am a fucking piece,” demonstrating her strength despite the horrors she has endured.
