When an off‑hand remark costs him a house‑surgeon slot to Dr. Bingham, young Dr. Simon Sparrow drifts between temporary posts. In a remote country clinic he discovers he once treated the husband of a grieving patient, and later, at a city hospital, he examines a precociously mature teenager while courting the diligent nurse Nan McPherson.

Back at St Swithin’s, Dr Simon Sparrow loses out to the self-important Dr Charles Bingham for a post as senior house surgeon. Convinced he has little future in surgery, he accepts a general practitioner role in an industrial town. There he shoulders most of the workload, including night calls, and endures the flirtatious advances of his partner’s wife.
He then takes a locum job with Dr Erasmus Potter-Shine’s Harley Street practice, where most patients are dotty aristocrats and neurotic society women. After three months, he moves to a rural practice where patients pay in kind, ranging from home-grown raspberries to poached salmon.
Meanwhile, Dr Tony Benskin fails his finals again and travels to Ireland where he buys a very dubious medical degree. This leads to a post as private physician to a rich elderly aristocratic lady in Wiltshire.
Sparrow and Benskin take a short holiday in France, where they rescue Charles Hopcroft, a governor at St Swithin’s, from an embarrassing incident. In gratitude, he arranges for Sparrow and Benskin to return to St Swithin’s. Sparrow commences advanced surgical training with Sir Lancelot Spratt, while Benskin becomes the personal physician to a wealthy Maharajah of Rhanda.
