A group of students from Byakudan Senior High School have just performed the “Sachiko Ever After” charm and, as a result, find themselves drawn to Tenjin Elementary School. The haunting welcome comes soon after the stage is set for tragedy: Mitsuki Yamamoto [Hitomi Noda] is murdered by Sachiko Shinozaki [Honoka Naito], a death that shatters the others and kicks off a chilling sequence of events.
The narrative then follows Naomi Nakashima [Rina Ikoma], who has spent months in a hospital bed after the traumatic losses of her friends. Ayumi Shinozaki [Nozomi Maeda] explains that her sister, Hinoe Shinozaki [Nijika Ishimori], has been researching ways to set things right, and Ayumi’s act of biting a bone once owned by Sachiko sends Naomi back in time to the haunted school. Naomi’s return is bittersweet, as she is overjoyed to see her best friend Seiko Shinohara [Yoko Kita] alive again, even as Ayumi admits that saving Yui and Mayu from their fates remains unlikely. Seiko and Naomi soon discover Sakutaro’s headless body, a grim sign that the cycle of deaths may be repeating itself.
Hinoe locates the missing bone and realizes Ayumi’s plan is flawed. Two crucial elements stand between another round of tragedy: the forbidden Book of Shadows and an entity known as “Sachi.” The tension tightens as Satoshi Mochida [Ryousuke Ikeoka] finds his little sister Yuka Mochida [Ayu Matsuura] together with Ayumi and Yoshiki Kishinuma [Jun], but Sachiko’s presence hommevers them once again. Naomi uses Sachiko’s mother’s necklace to repel Sachiko, buying a moment of safety for the group. Kensuke Kurosaki [Atomu Mizuishi] and Touko Kirisaki [Ren Ishikawa] hide, waiting for Yuuya Kizami [Tsunenori Aoki] to arrive and turn the tide of danger.
The survivors from Byakudan re-explore Sachiko’s corpse as they did previously, and tragedy strikes again when Yuka dies. Naomi places the protective necklace on Sachiko, nudging her toward a moment of sanity, and the crew races to return home. Yet the reverse charm proves inert, and a heated argument ensues as they confront the possibility that history cannot be rewritten so easily. Seiko is decapitated by a figure claiming to be Sachiko, and Sachi emerges from her body to finish off Touko.
Naomi, Kensuke, and Yuuya confront the grim truth through a found video camera: Emi Urabe [Rinka Ichishima] is seen crying as Yuuya stabs her to death. Kensuke demands answers, only to learn that Yuuya is not merely dangerous but deeply unhinged, responsible for the deaths of Ryosuke Katayama and Sakutaro. Hinoe intercedes to save Ayumi, Yoshiki, and Satoshi from the vengeful spirits, and explains that Sachi is Sachiko’s resentful twin sister who awakened when Sachiko rose again.
As the danger intensifies, Sachiko’s ascent marks a new level of threat. Yuuya lures the trio to a science room, but Satoshi discerns his deception and challenges him. In the ensuing struggle, Yoshiki dies, and a dying Satoshi fatally strikes Yuuya with a bottle of sulfuric acid, leaving his face scarred.
Hinoe then reveals the existence of the Book of Shadows, but a horribly scarred Yuuya kills her and continues the chase. Ayumi musters her courage, grabbing Sachi and dragging them both toward an uncertain fate. Naomi follows Sachiko’s voice to the book’s location, where a pale, white Sachiko appears. Naomi eventually finds herself back in the real world. She opens the Book and proclaims, “Please let everyone live!” The room trembles as the voices of her friends echo around her, and a grotesque creature—composed of the bodies of her dead friends, including Sachiko—crawls into the room, forcing Naomi into a terrifying cliffhanger. The book tears apart, leaving Naomi’s ultimate fate unknown.
