In the gritty, gang-ridden underbelly of Chicago's Southside, a war zone where violence has become a way of life, Demetrius Dupree (Chi-Raq) and his Spartans are locked in a bloody battle with their arch-nemesis, the Trojans, led by the ruthless Cyclops. Amidst the chaos, Demetrius's lover, Lysistrata, is driven to desperation as the relentless cycle of violence threatens to consume them all. The trigger for her transformation comes when a young girl, Patti, loses her life in a senseless gang shooting - an innocent victim caught in the crossfire of a war that shows no signs of abating.

As Lysistrata struggles to come to terms with the horror she has witnessed, she finds solace in the wise counsel of Miss Helen Worthy, a crusader for non-violence who introduces her to the inspiring story of Leymah Gbowee, a hero who brought an end to the Second Liberian Civil War through a bold sex strike. Energized by this newfound knowledge, Lysistrata sets out to mobilize the women of Chicago's Southside, rallying them to withhold their affection until the men agree to lay down their arms. The result is a citywide sex strike that rapidly gains momentum as women from all walks of life and every neighborhood join the boycott.

Despite the strike's massive membership, the Spartans and Trojans remain defiantly entrenched in their conflict. It takes the funeral of young Patti - a poignant reminder of the devastating human cost of their war - to stir Lysistrata into action once more. She turns to Fr. Mike Corridan, a passionate advocate for social justice, who implores her to confront the deeper issues driving the violence on Chicago's Southside. The realization dawns that the problem is far larger than the petty squabbles between rival gangs - it is a symptom of a broader societal rot.

Seizing the initiative, Lysistrata and her fellow women orchestrate an audacious coup by infiltrating a military armory and securing its contents from its unsuspecting guards. The takeover sparks a national crisis as the authorities scramble to respond to this unprecedented challenge. Faced with 75 unarmed women holding the fort, the military and police are at a loss for how to regain control. Meanwhile, Lysistrata's sex strike has galvanized a global movement, inspiring women from around the world to follow suit.

In a desperate bid to outmaneuver the women, Mayor McCloud and Commissioner Blades concoct a plan to lure them out of the armory with seductive music - only to be foiled when the women discover the military's earplugs and the men are left squirming in unrequited longing. The city teeters on the brink of chaos as Lysistrata's rebellion continues to spread, refusing to back down until her demands for peace are met.

As the initial fervor of the Spartans' rebellion begins to wane, the absence of intimate connections and an abundance of time to reflect on their circumstances take a toll on both Cyclops's and Demetrius's gangs. Despite his unwavering dedication to the Spartans, Demetrius finally concedes to Lysistrata's persistence and agrees to meet with her in a high-stakes game of wits, where the winner will dictate the terms of the other's surrender. The stakes are elevated when the city brokers a deal: whichever party reaches climax first must yield to the other's demands. However, this delicate balance is disrupted by Cyclops' sudden arrival, as he announces that the Trojans are laying down their arms.

The following day, Lysistrata, Mayor McCloud, and Cyclops come together to sign a historic accord aimed at ending the cycle of violence and building a foundation for healing. Demetrius, however, refuses to surrender his ideals and walks away from the agreement, but his resolve is eventually shaken by the poignant testimony of Miss Worthy, who recounts the tragic loss of her daughter, Irene, whose death was orchestrated by none other than Demetrius's own father, Jamel Dupree. As the weight of his complicity settles in, Demetrius confesses to being the one responsible for Irene's daughter's demise and submits to arrest, marking a turning point in his journey towards redemption.
