Mark Kaminski Arnold Schwarzenegger is a small-town sheriff who lives with his alcoholic wife Amy Kathryn Harrold. He is a former FBI agent who once brutalized a suspect accused of sexually assaulting and murdering a young girl, a memory that haunts him after a prosecutor, Marvin Baxter, forces him to choose between resignation or prosecution and ultimately suspends him. In the wake of a deadly ambush that wipes out a protective detail for a mob informant, Blair Shannon, the son of FBI agent Harry Shannon, is killed. The case that sparks Kaminski’s break from the ordinary starts when Special Federal Prosecutor Baxter initiates a sweeping inquiry into Luigi Patrovita, the most formidable figure in the Chicago Outfit’s operations. A leak within the FBI is blamed for the killings that have begun to plague Patrovita’s world, and the fallout forces the hand of the agency to act with a unique kind of recklessness. In response, Shannon recruits Kaminski for an unsanctioned, covert mission: infiltrate Patrovita’s organization from within and help dismantle it. Kaminski fakes his own death in a chemical plant explosion and adopts the alias Joseph P. Brenner, a convicted felon, as his cover. The ruse allows him to slip into Patrovita’s orbit and begin earning the mob’s trust.

Inside the casino’s basement under the high-class hotel, Kaminski encounters Paulo Rocca, Patrovita’s right-hand man, and he wins his favor by targeting a rival boss, Martin Lamanski, who’s trying to push into Patrovita’s territory. Kaminski’s boldness and calculated misdirection gradually gain him access to Patrovita’s inner circle, a dangerous place where every move could blow his cover. In this web of deception, Kaminski also meets Monique, a woman who works for Rocca’s top lieutenant Max Keller. The early camaraderie and shared peril reveal a man who is willing to risk everything to strike at the heart of the mob’s operation.

As Kaminski plays both sides, Keller grows suspicious that Brenner may not be who he seems, and he procures proof by showing Kaminski’s photo to a police informant who once arrested the real Brenner. The leak within Baxter is exposed, forcing Baxter to stay close to Patrovita as the investigation continues. Kaminski and Monique share moments that hint at more than just a professional alliance, and the mission’s tension intensifies when a cemetery hit operation takes a shocking turn: Kaminski’s cover nearly comes undone, and he’s compelled to confront Keller directly, killing him in a tense exchange that leaves Kerry’s side in disarray and Blair Shannon’s father reeling from the escalation.

Kaminski’s escape is aided by Monique, who urges him to head to the airport and wait for orders. Back on the offensive, he assembles an arsenal of guns and ammunition and launches a devastating raid on Patrovita’s gravel pit, slaughtering the mob’s guards and siphoning off a massive amount of heroin and cash. With new firepower at his disposal, Kaminski makes his way to Patrovita’s casino, initiating a ruthless spree that wipes out the mob’s soldiers one by one. Rocca and Patrovita retreat into a back room, but Rocca falls first to the onslaught, leaving Patrovita alone to face his fate.

Patrovita tries to plead for mercy in an office, but Kaminski does not waver and shoots him dead. As he exits, he comes face to face with Baxter, who attempts to apologize for the suspension that catalyzed so much bloodshed. Kaminski’s response is cold and decisive: a reminder of Baxter’s earlier demand—“Resign, or be prosecuted. Any way you want it.” >Resign, or be prosecuted. Any way you want it.< Kaminski shoots Baxter in what he calls self-defense, signaling that the consequences of the fallout are now his burden to bear.

With the mission accomplished and Patrovita’s organization dismantled, Kaminski drives to the airport and hands Monique a duffel bag filled with $250,000 in cash, suggesting she can start a new life free from obligations. Back in formal channels, Kaminski is reinstated by the FBI and is reunited with his pregnant wife, Amy. In a last act of gratitude and healing, Kaminski visits Blair Shannon’s father, Harry Shannon, who has resisted physical therapy. Kaminski asks Shannon to be the godfather of his unborn child in exchange for finishing the therapy, and Shannon accepts, a quiet pledge that the costs of vengeance have given way to the promise of family and closure.
