In 1854, Puang, a soldier wounded in battle, encounters the Black Spirit Manita Chobchuen, a woman dressed in black who offers to keep him alive in exchange for serving as her host. After Puang agrees, she removes her tongue and presses it into his mouth. The ghoul, Puang’s captive host, has since controlled Puang and claimed many victims over the years, turning his fate into a long, dark burden that haunts the present.
Fast forward to 1975, three years after Puang’s ghoul caused Yam’s death in the first film, Nadech Kugimiya as Yak has surrendered everything to become a hunter of evil supernatural beings, driven by a deep need to avenge his sister. He is not alone: he is joined by his loyal ally, Sarge Ongart Cheamcharoenpornkul as Sergeant Paphan, who has managed to survive the carnage from the previous story. Their shared mission drags them toward the haunted wilderness of Dong Khomot, a forest where a chorus of malevolent spirits and dark entities makes their task feel almost unwinnable.
In the forest, they are joined by five others: Luechai, a man who has also lost family members to the ghoul; Jirasak, portrayed by Jampar Saenprom as Jirasak; Nawin, played by Samiha Lamloet Haque as Nawin; Sem, a necromancer from Laplae; and Khom, a porter from Northern Thailand. Together they press on, facing a succession of malignant beings that block their path. Sem is killed by a venomous king cobra that sprawls across their way, and Jirasak, terrified by Sem’s ghost, opens fire on the group, killing Khom and then ultimately shooting off his own head. Nawin is pulled away from the group and is later devoured by a werecat, which briefly assumes Nawin’s form to lure the others toward danger.
Only Yak, Sarge, and Luechai remain, and they manage to squeeze into a salt lick—the sacred ground that may offer a slim chance at sanctuary. The next morning, they reach a dilapidated hut near a swamp and confront Puang, who has endured over a century as the ghoul’s immortal host. Puang explains that a ritual long ago had failed because Luechai’s ancestors interrupted it by burning down his hut. Over the years Puang has hidden from the ghoul thanks to a protective amulet on his arm. When Luechai cuts off Puang’s arm, the ghoul materializes, killing Luechai and Puang and reclaiming her tongue. Yak attempts to fight, but the ghoul compels him to shoot himself in the chest, seemingly ending his life.
Meanwhile, back at a hotel where Yak’s family has gathered for Yad’s upcoming wedding to Pradit, the ghoul makes her presence felt in the most insidious ways. She enters the hotel and sows confusion and sleepiness with whispering voices. Pradit succumbs to her control and attacks Yak’s father and brothers, who manage to subdue him with a Buddha figurine. The ghoul also seizes Boonyen, forcing her to hunt Yee with a gleaming pickaxe. Yee dodges Boonyen’s possession, while Chauy’s ghost—also under the ghoul’s sway—looms nearby. Yad, separated from her mother and sister, experiences a chilling vision of Yam before the ghoul strikes again.
Then Yak and Sarge reappear to confront the ghoul head-on. Puang’s amulet and a voodoo doll that he created grant Yak a remarkable edge: wearing Puang’s amulet makes him invisible to the ghoul as they clash. The ghoul grabs Yad and threatens to hurl her from a window unless Yak reveals himself by removing the amulet. He complies, but Yad, wearing Puang’s amulet herself, pulls out the ghoul’s tongue, allowing Yak to cut it off and giving Sarge the chance to destroy the severed tongue. Weakened without her tongue, the ghoul is cornered and finally vanquished when Yak pounds her forehead with a takrut, freeing all the ghosts of her victims who had been kept under her control.
With the threat ended, the family is reunited, and Yak sees Yam’s ghost smiling at him, a quiet, bittersweet sign that some losses can finally begin to heal even as the living move forward.

The group enters Dong Khomot forest
Yak, Sarge, and their companions enter the Dong Khomot forest, a realm where many malevolent beings reside. They are joined by Luechai, his nephews Jirasak and Nawin, Sem the necromancer, and Khom the porter. The forest tests their nerves as they push deeper toward the ghoul.
Sem killed by a king cobra
A king cobra blocks their path and killing Sem underlines the forest’s lethal dangers. The group regroups, aware that every step could bring a supernatural threat. The incident foreshadows the peril that lies ahead.
Jirasak panics, violence erupts
Terrified by Sem's ghost, Jirasak panics and opens fire on the group, killing Khom and then shooting at his own head. The trust within the group collapses as fear spreads. The survivors struggle to proceed with the mounting chaos.
Nawin devoured by a werecat
Nawin is separated from the others and is devoured by a werecat, which later imitates his form to lure the living toward doom. The loss deepens the danger and heightens the sense that the forest itself is guiding their fate. Yak, Sarge, and Luechai press on with caution.
Trio reaches sacred salt lick and meets Puang
Yak, Sarge, and Luechai reach a salt lick considered sacred ground. The next morning they encounter Puang, alive after more than a century, bound as the ghoul's host. Puang explains how an earlier ritual was interrupted by Luechai's ancestors and how his amulet keeps him hidden from the ghoul.
Puang's arm severed; ghoul emerges
Seeking to expose Puang and end the binding, Luechai cuts off Puang's arm. The ghoul appears, kills Luechai and Puang, and retrieves her tongue, strengthening her grip on the scene. Puang hints that the ancient ritual failure left him alive but vulnerable to her power.
Ghoul invades the hotel for Yad's wedding
While the forest confrontation continues, the ghoul travels to Yak's family hotel for Yad's wedding, spreading a drowsy whisper to lull the guests. Pradit is possessed and attacks the family until they knock him out with a Buddha figurine. The wedding becomes a battleground between the living and the supernatural.
Boonyen and Yee hunted; Yad's vision
The ghoul possesses Boonyen and sends her to hunt Yee with a pickaxe. Yee evades Boonyen and Chauy's ghost, both under the ghoul's control. Yad, separated from her family, experiences a vision of Yam before the ghoul strikes again.
Yak and Sarge confront the ghoul; invisibility with amulet
Yak and Sarge face the ghoul in the hotel, with Puang's amulet rendering Yak invisible to her during the fight. Yad also wears Puang's amulet to aid in the confrontation. They struggle to hold the line as the ghoul presses her advantage.
The ghoul demands Yak reveal himself
The ghoul grabs Yad and threatens to throw her from a window unless Yak removes the amulet and reveals himself. Yak agrees to expose himself to save Yad, buying time as Sarge braces for the worst. The standoff escalates the danger of the living against the supernatural.
Tongue extraction and final destruction
Yad, wearing Puang's amulet, pulls out the ghoul's tongue, allowing Yak to cut it off. Sarge destroys the severed tongue, weakening the ghoul until she is cornered. Yak pounds the takrut on her forehead, freeing the ghosts of her victims.
