Picking up after The Mad Doctor of Blood Island, the story centers on Dr. Bill Foster John Ashley and a crew that includes Sheila Willard, her father, and Carlos Lopez, when a mut­ated chlorophyll creature—the Beast, Don Ramon Lopez—secretly aboard a ship they travel on explodes and goes on a ruthless rampage. The monstrous Don Ramon washes ashore on Blood Island and disappears into the jungle, leaving Foster as the sole survivor. After several months of recovery in a hospital, he returns to Blood Island on another voyage, this time accompanied by reporter Myra Russell Celeste Yarnall, who is hot on the trail of the ship’s destruction.

When Foster and Myra land, the locals are gripped by fear, convinced that the old Lopez mansion is cursed. The village headman Ramu Alfonso Carvajal and the ship’s captain Beverly Miller lead a search of the mansion and uncover Razak Bruno Punzalan, who is alive. The enigmatic “green men” reappear, Myra is captured, and she is brought to the scarred but still living Dr. Lorca Eddie Garcia, a figure thought to have died in the previous film.

Foster teams up with Laida Liza Belmonte to scale the mountains and locate Lorca’s hidden lab. Laida returns to the village to summon help as Lorca unveils his latest experiments: he has removed Don Ramon’s head and attached it to machines, while the decapitated body remains strapped to a table. The head can speak, but it refuses to divulge anything.

The captain, Laida, and Ramu lead an assault on Lorca’s headquarters and clash with his men. Laida finds her father showing signs of chlorophyll poisoning, and Foster shoots Razak to stop him from causing more trouble.
