After an unknown outbreak devastates the United States, a family of three—Ray, Claire, and their daughter Zoe—takes refuge in an abandoned fallout shelter as the Air Force begins bombing their town. The trio clings to a strict set of rules designed to keep them alive: stay calm, never leave the shelter, and keep each other safe at all costs. Yet danger stalks them from beyond the hatch.
In the claustrophobic silence, a rat discovers the dwindling food supply and becomes a tense, unwelcome challenge. The family works to kill it, only to learn the rodent found its way in through a small opening other than the barricaded hatch. They discover a narrow air duct and, in a moment of stubborn reassurance, Ray tells Zoe that the Breathers—the nightmarish figures outside—won’t be able to squeeze through. For a while, it seems they might be the last people left alive.
The threat from the outside intensifies as the Breathers close in on the shelter. Claire ends up killing the rat, but a careless accident sends a lamp tumbling and bursting into flames. The smoke rises up the duct, creeping toward the shelter and slowly exposing their position. When the smoke deposits ash on the hatch, their hiding place becomes visible to any lurking danger.
Desperation grows, and the family decides to venture to the surface to cover the shelter’s opening. Zoe, armed with a homemade periscope, spots a distant humanoid figure, and against the rulebook, she climbs through the barricade to warn her parents. They manage to retreat back inside, but the encounter leaves them vulnerable as a Breather follows.
A flashback reveals the outbreak’s chilling dawn: Ray, Claire, and Zoe race along a highway, only to be halted by a quarantine checkpoint manned by the CDC. The world outside is collapsing, and signs of containment multiply as the family witnesses the beginning of a grim new reality.
Back in the present, the Breathers grow bolder and more numerous, ultimately breaking through the shelter hatch. Claire and Zoe flee to the air duct, while Ray is captured and disappears, his fate marked by a spray of blood. On the highway, they are recaptured by American soldiers wearing night-vision gear and re-breathers, who surround them with rifles drawn.
Another flashback shows two military jets roaring overhead as they bomb Kingsville. The family hurries to an abandoned shelter, where Claire injures herself on rusted metal. The injury accelerates the realization that the virus is airborne and they are already infected, a truth that makes every breath a risk.
In the present, soldiers test Claire and Zoe’s blood, interrogating them for nearby survivors before preparing to execute them. Out of nowhere, Ray reappears, killing one soldier and incapacitating several more with startling speed and strength as he is shot repeatedly and falls. Claire and Zoe break free from their net, and, in a fierce display of drive, Claire kills more soldiers while Zoe—in a furious, protective state—exposes a soldier to the air by removing his re-breather, infecting him. The two of them barely hold back from harming each other as Claire reminds Zoe to keep calm and adhere to their rules.
Ray’s collapse signals the end of their immediate struggle, and the trio seeks shelter in a sewer system. They huddle in the damp tunnels, listening for any approaching threat, until a familiar voice answers their calls: Joey, Zoe’s neighbor and friend. He has survived and leads them to others who endure in the underground network. The group chooses to join this growing community of infected survivors as dawn breaks.
As the sun rises, Zoe notes that they’ve endured day 302. Claire offers a different perspective, reframing the math of survival: Not days, but miracles. The two climb toward a sewer hatch and watch the world outside brighten through a window, with Claire’s eyes bloodshot and unyielding as she exits into the rising light with Zoe, ready to face whatever comes next in this shattered new world.

Outbreak forces family into shelter
An unknown, deadly outbreak devastates the United States. Ray, Claire, and their daughter Zoe seek refuge in an abandoned fallout shelter as military bombings escalate. Inside, they establish basic rules to survive the claustrophobic conditions.
Rules become their survival routine
With dwindling resources, the family imposes strict rules to stay calm, keep quiet, and avoid drawing attention. They practice daily routines to create a sense of normalcy in a fragile world. The shelter becomes their only hope of safety.
Rations dwindle as a rat finds its meal
During dinner, they discover a rat has been consuming their remaining rations. The family must decide how to respond while facing starvation and fear. They set out to locate and kill the rat to protect what little food remains.
Duct discovery and a false sense of safety
The trio finds a small air duct and Ray reassures Zoe that the Breathers wouldn’t fit through it. This belief reinforces their sense of security and the plan to stay hidden. The discovery hints at a potential escape if needed.
Zoe's nightmare signals the danger
Zoe wakes from a nightmare about the Breathers, underscoring the constant fear in confinement. The family tries to rationalize the danger, but the dream foreshadows looming threat. The moment deepens the emotional strain of their shelter life.
Smoke reveals their position
Claire accidentally burns a wooden table while killing the rat, and the resulting smoke drifts up the air duct. The smoke and ash visibly cover the shelter hatch, making their hiding place easy to detect. Realizing their risk, they prepare to surface to cover the shelter.
Zoe spots a Breather from the surface
Using a homemade periscope, Zoe spots a humanoid figure in the distance on the surface. The family retreats back to safety as the threat lingers nearby. They decide to keep quiet and wait for the danger to pass.
Zoe defies rules to warn her parents
Disregarding the rule to stay quiet, Zoe climbs through the barricade to warn her parents of the Breather's presence. The Breather begins to break in, and the family scrambles to stay hidden. They narrowly avoid detection for the moment.
Flashback: the highway and quarantine
In a flashback, Ray, Claire, and Zoe drive along a highway trying to flee Kingsville, only to be stopped by the CDC. Signs announce a strict quarantine, underscoring a world collapsing under containment. The moment contrasts with their current desperation inside the shelter.
Jets strike and the virus is revealed
A bomb blast punctuates the early outbreak, and the family rushes to an abandoned shelter as jets destroy their town. They realize the virus is airborne and that they may already be infected. This knowledge raises the stakes for every decision they make.
Breathers breach and Ray disappears
More Breathers arrive and force their way through the shelter hatch. Claire and Zoe retreat into the air duct, but Ray is captured and disappears amid a spray of blood. The encounter pushes the family toward the dangerous world beyond their shelter.
Interrogation, rescue, and a brutal fight
Soldiers test Claire and Zoe's blood and prepare to execute them. Ray suddenly appears, killing one soldier and displaying extraordinary strength before he is shot and falls. The family is left in a precarious, violent stand-off with the outside world.
Claire's ferocity and Zoe's infection move
Claire undergoes a transformation akin to Ray, attacking soldiers with feral intensity and killing several. Before the last soldier can kill Claire, a transformed Zoe exposes him to air, infecting him. Zoe's rage tests the limits of their resolve, but Claire urges calm to maintain order.
Escape to the sewer and meet survivors
Claire and Zoe manage to slip free and flee toward a sewer system beneath the town. They meet Joey, Zoe's neighbor, who has survived and leads them to others like them in the underground tunnels. The group begins to reconcile their uncertain new reality.
Day 302 and a fragile future
Zoe notes that they have survived to day 302, suggesting miracles rather than a schedule. Claire looks up toward a sunlit window with bloodshot eyes, signaling infection’s toll and the uncertain fate ahead. They join the other infected survivors in the sewer, facing a long, uncertain future.
